Introdução: A qualidade de vida relacionada à saúde (QVRS) constitui um importante indicador em indivíduos com obesidade, especialmente no contexto da cirurgia bariátrica, procedimento associado a mudanças significativas nos aspectos físicos, psicológicos e sociais. Objetivo: Investigar a qualidade de vida relacionada à saúde e sua associação com fatores sociodemográficos, comportamentais e clínicos em adultos candidatos e submetidos (=12 meses e =13 meses) à cirurgia bariátrica. Métodos: Este foi um estudo transversal, conduzido com pacientes atendidos entre janeiro de 2022 e março de 2026, no Hospital Universitário da Universidade Federal de Santa Catarina. A coleta de dados foi realizada por meio de questionário. O desfecho foi a QVRS, avaliada pelo instrumento Impact of Weight on Quality of Life-Lite (IWQOL-Lite), composto pelos domínios função física, autoestima, vida sexual, constrangimento público e trabalho, avaliada de forma contínua e categorizada como muito ruim (0?20), ruim (21?40), regular (41?60), boa (61?80) e muito boa (81?100). As exposições incluíram fatores sociodemográficos, comportamentais e clínicos. Utilizou-se regressão linear múltipla, com estimativas expressas por coeficientes ß, intervalos de confiança de 95% (IC95%) e coeficiente de determinação ajustado (R² ajustado). Resultados: Participaram do estudo 173 indivíduos, sendo 16,2% no período pré-cirurgia bariátrica, 26,6% no pós-cirurgia bariátrica =12 meses e 57,2% no pós-cirurgia bariátrica =13 meses. Indivíduos no período pré-cirurgia apresentaram menores pontuações de QVRS global [44,0 (32,9?52,0)] quando comparados aos grupos =12 meses [71,4 (60,3?86,1)] e =13 meses [83,1 (71,4?91,5)] (p<0,001). Esse padrão também foi observado em todos os domínios avaliados. Enquanto a maioria dos pacientes pré-cirurgia (60,7%) apresentou a QVRS muito ruim/ruim, nos pacientes pós-cirurgia bariátrica (=12 meses e =13 meses), a QVRS foi classificada, predominantemente, como boa/muito boa (69,6% e 81,8%, respectivamente). Em pacientes pré-cirurgia bariátrica, a QVRS global associou-se de forma direta com uma maior escolaridade [ß = 16,7 (4,9; 28,5)] e com a prática de atividade física = 150 min/sem [ß = 17,9 (5,7; 30,1)], e de forma inversa com o índice de massa corporal [ß = -0,7 (-1,4; -0,5)]. Em pacientes pós-cirurgia bariátrica (=12 meses), a QVRS global associou-se de forma direta com a escolaridade [ß = 12,4 (0,1; 24,6)] e com a autopercepção de saúde [ß = 6,9 (0,8; 13,1)] e de forma inversa com o índice de massa corporal [ß = -1,3 (-2,1; -0,6)]. Em pacientes pós-cirurgia bariátrica (=13 meses), a QVRS global associou-se à prática de atividade física no deslocamento [ß = 10,6 (2,2; 18,9)] e com autopercepção de saúde [ß = 9,3 (4,9; 13,7)]. Quando os domínios da QVRS foram analisados, observou-se mais expressivamente que a função física esteve inversamente associada à idade [ß = -0,6 (-1,2; -0,1) e -0,6 (-0,9; 0,1)] e diretamente associada à autopercepção de saúde [ß = 9,6 (1,9; 17,4) e 10,1 (5,5; 14,7)] em pacientes pós-cirurgia bariátrica (=12 meses e =13 meses, respectivamente). A vida sexual associou-se diretamente com a escolaridade em pacientes pré e pós-cirurgia bariátrica (=12 meses) [ß = 27,5 (7,3; 47,7) e 28,4 (9,6; 47,1), respectivamente]. O constrangimento público foi inversamente associado ao IMC nos grupos pós-cirurgia bariátrica =12 meses e =13 meses, respectivamente [ß = -1,8 (-3,5; -0,4); -1,9 (-2,9; -0,9)]. Já no domínio da QVRS no trabalho, observou-se associação direta com a autopercepção de saúde [ß = 10,2 (1,4; 19,1) e 12,2 (6,5;18,0), pós =12 meses e =13 meses, respectivamente]. Conclusão: Foram observadas diferenças na QVRS global entre adultos candidatos e submetidos à cirurgia bariátrica. Além disso, as associações entre fatores sociodemográficos, comportamentais e clínicos e a QVRS global variaram conforme o período em relação à cirurgia bariátrica, destacando-se maior escolaridade, prática de atividade física, menores valores de IMC e melhor autopercepção de saúde. Abstract: Introduction: Health-related quality of life (HRQoL) is an important indicator in individuals with obesity, particularly in the context of bariatric surgery, a procedure associated with significant changes in physical, psychological, and social aspects. Objective: To investigate health-related quality of life and its association with sociodemographic, behavioral, and clinical factors in adults awaiting bariatric surgery and those who had undergone bariatric surgery (=12 months and =13 months). Methods: This was a cross-sectional study conducted with patients treated between January 2022 and March 2026 at the University Hospital of the Federal University of Santa Catarina. Data were collected using a structured questionnaire. The outcome was HRQoL, assessed using the Impact of Weight on Quality of Life-Lite (IWQOL-Lite) questionnaire, comprising the domains of physical function, self-esteem, sexual life, public distress, and work. HRQoL was analyzed as both a continuous variable and categorized as very poor (0?20), poor (21?40), fair (41?60), good (61?80), and very good (81?100). The exposures included sociodemographic, behavioral, and clinical factors. Multiple linear regression analyses were performed, with estimates expressed as ß coefficients, 95% confidence intervals (95% CI), and adjusted coefficient of determination (adjusted R²). Results: A total of 173 individuals participated in the study, of whom 16.2% were in the pre-bariatric surgery period, 26.6% were within =12 months after bariatric surgery, and 57.2% were =13 months after bariatric surgery. Individuals in the pre-bariatric surgery period presented lower overall HRQoL scores [44.0 (32.9?52.0)] than those in the =12-month [71.4 (60.3?86.1)] and =13-month [83.1 (71.4?91.5)] post-bariatric surgery groups (p<0.001). This pattern was also observed across all HRQoL domains. Whereas most pre-bariatric surgery patients (60.7%) were classified as having very poor/poor HRQoL, most post-bariatric surgery patients (=12 months and =13 months) were classified as having good/very good HRQoL (69.6% and 81.8%, respectively). Among individuals in the pre-bariatric surgery group, overall HRQoL was positively associated with higher educational level [ß = 16.7 (4.9; 28.5)] and engagement in physical activity =150 min/week [ß = 17.9 (5.7; 30.1)], and inversely associated with body mass index [ß = -0.7 (-1.4; -0.5)]. Among individuals =12 months after bariatric surgery, overall HRQoL was positively associated with educational level [ß = 12.4 (0.1; 24.6)] and self-rated health [ß = 6.9 (0.8; 13.1)], and inversely associated with body mass index [ß = -1.3 (-2.1; -0.6)]. Among individuals =13 months after bariatric surgery, overall HRQoL was positively associated with transport-related physical activity [ß = 10.6 (2.2; 18.9)] and self-rated health [ß = 9.3 (4.9; 13.7)]. When the HRQoL domains were analyzed, physical function showed the strongest associations, being inversely associated with age [ß = -0.6 (-1.2; -0.1) and -0.6 (-0.9; 0.1)] and positively associated with self-rated health [ß = 9.6 (1.9; 17.4) and 10.1 (5.5; 14.7)] among individuals =12 months and =13 months after bariatric surgery, respectively. Sexual life was positively associated with educational level among individuals in the pre-bariatric surgery group and those =12 months after bariatric surgery [ß = 27.5 (7.3; 47.7) and 28.4 (9.6; 47.1), respectively]. The public distress domain was inversely associated with BMI in the =12-month and =13-month post-bariatric surgery groups [ß = -1.8 (-3.5; -0.4) and -1.9 (-2.9; -0.9), respectively]. In the work domain, a positive association was observed with self-rated health [ß = 10.2 (1.4; 19.1) and 12.2 (6.5; 18.0) in the =12-month and =13-month post-bariatric surgery groups, respectively]. Conclusion: Differences in overall HRQoL were observed between adults awaiting bariatric surgery and those who had undergone the procedure. Furthermore, the associations between sociodemographic, behavioral, and clinical factors and overall HRQoL varied according to the period in relation to bariatric surgery, with higher educational level, physical activity, lower BMI, and better self-rated health emerging as the main associated factors. |
