Políticas públicas: um olhar para a saúde bucal das pessoas com deficiência visual

Abstract

Com a Constituição Federal de 1988, foram criadas políticas de inclusão para pessoas com deficiências que asseguram direitos em diversos setores sociais. Por meio do Sistema Único de Saúde (SUS) é ofertada à população a assistência e cuidados em saúde, sendo a saúde bucal (SB) parte desse cuidado. Segundo a Lei nº 7.853, (1989) a deficiência visual, seja ela cegueira total ou baixa acuidade visual, é considerada deficiência por gerar incapacidade de exercer algumas atividades dentro do padrão considerado para o ser humano. Dessa forma têm-se as políticas de inclusão abrangem pessoas com deficiência visual (DV). A pesquisa objetivou analisar a percepção do DV frente à atenção à SB recebida de cirurgião-dentista (CD) em relação ao preconizado pelas políticas públicas. Trabalho de abordagem qualitativa que ocorreu na Associação Catarinense para Integração dos Cegos (ACIC) e teve como participantes de pesquisa DV matriculados no ano de 2017. A coleta dos dados foi realizada após a aprovação do projeto de pesquisa pelo Comitê de Ética em Pesquisa com Seres Humanos (CEPSH) na Universidade Federal de Santa Catarina (UFSC), com aceitação e a assinatura do Termo de Consentimento Livre e Esclarecido (TECLE) pelos participantes. Realizou-se entrevista semiestruturada com respostas livres, gravadas e transcritas posteriormente. Para a análise de conteúdo foi utilizada a técnica de Minayo. Os resultados apresentaram seis temáticas que foram agrupadas em três categorias: O DV e o autocuidado em SB; A relação do DV com o profissional de saúde; A relação do DV com o serviço de saúde ofertado. Visando o conhecimento do DV frente aos cuidados com a higiene bucal foi identificada a necessidade de ações de orientação em saúde que utilizem a linguagem tátil-sensorial para desenvolver habilidades e percepção da qualidade da escovação. Na categoria referente ao CD e seu contato com o DV foi identificada uma boa relação pautada na confiança e empatia, porém, ainda há falhas de comunicação entre os lados. É preciso que o CD seja capaz de entender a linguagem do DV e também se expressar de forma compreensível para que ocorra um tratamento adequado e criação de vínculo. Em relação ao serviço de saúde, notou-se que os DV têm acesso ao atendimento odontológico na rede básica de saúde com agilidade na marcação e são assistidos nos serviços de referência, considerando o atendimento resolutivo. Portanto, os achados desta pesquisa revelam que os CD vêm seguindo as orientações das políticas públicas para incluir e garantir o acesso dos DV nos serviços de saúde, porém necessitam de capacitação voltada a melhorar a comunicação.

With the Brazilian Constitution of 1988, inclusion policies were created for people with disabilities ensuring rights in various social sectors. Through the Unified Health System, health care is offered to the population, including oral health, based on the principles of universality, integrality, equity and decentralization of services. According to the law 7,853 (1989), the visual impairment, being it a total blindness or a low visual acuity, is considered deficiency because it generates to the person who carries it “loss of a structure or function that turn him unable to perform the activity within the standard considered for the human being... ". It means that the population with this deficiency belongs to inclusion policies. The aim of this research was to analyze the perception of the visually impaired about the oral health care received by dentists in relation to the recommended by public policies. A qualitative approach occurred at the Catarinense Association for the Integration of the Blind (ACIC) and had as subjects of the research the visually impaired persons enrolled in the year 2017. The data collection was done after approval of the ethics committee, with acceptance and the signing of the Human Informed Consent Form by the participants. A semi-structured interview was performed with free responses, recorded and transcribed later. The thematic analysis, proposed by Minayo, was used as data analysis technique. The results presented six themes that were grouped into three categories: visual impairment and self-care in oral health; The relationship of the visually impaired to the health professional; The relation of the visually impaired to the offered service. In search of the autonomy of the visually impaired person regarding oral hygiene care, identified the need for actions health guidance that use the tactile-sensory language for the visual impairment to "feel" the quality of the brushing and thus develop skills to keep the oral hygiene. In the category referring the dentist and his contact with the visually impaired, a good relationship based on trust and empathy was identified. However, there are still gaps in communication. The professional must be able to understand the visually impaired as well as to express himself comprehensibly to the patient to offer an appropriate treatment and create a real bond. Regarding the health service, it was noted that the visually impaired have access to dental care in the basic health network with agility in marking. They are also assisted in referral services. Considering these aspects, the patients report that they have their needs supplied by the services offered. Therefore, according to the participants, oral health professionals have followed the guidelines of public policies to include and ensure the access of the disabled population to the health services even if they need the training to improve the communication between them.
