Recomendações para o cuidado de enfermagem ao paciente no pós-operatório imediato de transplante de fígado em terapia intensiva

Abstract

No seu processo pós-operatório imediato, os pacientes submetidos a transplante de fígado são admitidos na Unidade de Terapia Intensiva. São pacientes submetidos a cirurgia de grande porte, com grande repercussão sistêmica e vulneráveis à instabilidade em diferentes sistemas orgânicos. Dentre as responsabilidades do enfermeiro intensivista estão planejar e implementar intervenções de enfermagem, sendo necessário para tanto possuir conhecimento específico para cuidar do paciente com este perfil, com base em ações fundamentadas e atuais. O presente estudo teve por objetivos identificar e avaliar as evidências disponíveis na literatura e elaborar um guia de recomendações para o cuidado de enfermagem baseado em evidências ao paciente submetido a transplante de fígado na Unidade de Terapia Intensiva. Foi utilizado o método de revisão integrativa da literatura, com a consulta as bases de dados Pubmed/Medline, LILACS, CINAHL e SCOPUS. A amostra foi composta por 18 estudos. Em relação ao nível de evidência, oito estudos eram de nível IV (coorte). Ao sintetizar as evidências identificadas nos estudos selecionados, foram destacados temas como o controle glicêmico, o controle de infecção, a ocorrência de injúria renal aguda e eventos neurológicos no paciente submetido a transplante hepático. Com base nos temas identificados e considerando outros dez estudos de revisão encontrados, o escopo foi ampliado, resultando no desenvolvimento de recomendações baseadas em evidências para assistência de enfermagem ao paciente no pós-operatório imediato de transplante de fígado em terapia intensiva. Espera-se que os resultados que emergiram deste estudo possam contribuir para a construção do conhecimento na área e para uma fundamentação maior da prática, consequentemente melhorando a qualidade da assistência prestada ao paciente submetido a transplante de fígado.<br>

Abstract : In the immediate postoperative process, the patients that undergo liver transplant are admitted to the Intensive Care Unit. They are patients that undergo major surgery, with major systemic effects and who are vulnerable to the instability in different organ systems. Among the ICU nurse's responsibilities are the planning and implementation of nursing interventions, so they need to have specific knowledge to take care of a patient with that profile, based on substantiated and current actions. The present study aimed at identifying and evaluating the evidence available in the literature and elaborating a recommendation guide for nursing care based on the evidence to the patient who undergoes liver transplant at the Intensive Care Unit. The method used was an integrative literature review, consulting the Pubmed/Medline, LILACS, CINAHL and SCOPUS data bases. The sample was composed by 18 studies. Regarding the level of evidence, eight studies were level IV (cohort). When synthesizing the evidence identified in the selected studies, the themes highlighted were the ones such as glycemic control, infection control, acute kidney injury and neurological events in the patient that has undergone liver transplant. Based on the identified themes and considering ten other review studies found, the scope was amplified, resulting in the development of recommendations based on evidences to help nursing the patient in the immediate postoperative period of liver transplant in intensive care. The results that emerged from this study are expected to contribute to build knowledge in the field and to better ground this practice, improving the quality of the assistance provided to the patient that has undergone liver transplant.
