A fibromialgia (FM) é definida como uma síndrome dolorosa crônica, caracterizada pela presença de dor musculoesquelética generalizada, com etiopatogenia pouco clara, frequentemente associada a sintomas de fadiga, insônia, distúrbios funcionais, alterações cognitivas e quadros psiquiátricos, ocorrendo predominantemente em mulheres. Sustentada pelo posicionamento epistemológico do Pensamento Complexo, esta pesquisa qualitativa teve como objetivo compreender os significados atribuídos à experiência de viver e conviver com a dor crônica na perspectiva de mulheres adultas diagnosticadas com FM. Participaram da pesquisa 12 (doze) mulheres com diagnóstico de FM. Para a coleta de dados, foram utilizados um questionário sociodemográfico e clínico e o recurso de entrevista semiestruturada em profundidade. O conteúdo dos dados foi organizado e analisado a partir do processo de codificação proposto pela Teoria Fundamentada nos Dados ? TFD (Grounded Theory ? GT), com o auxílio do software para dados qualitativos Atlas.ti 23. Os resultados foram dispostos em três artigos, sendo um artigo teórico e dois empíricos. O primeiro artigo foi uma revisão integrativa, que buscou compreender como a literatura científica aborda a interface da FM em mulheres e psicologia. Os resultados da revisão evidenciaram o impacto negativo da FM nas diferentes áreas da vida e a necessidade de uma maior produção de estudos qualitativos que abordassem as repercussões no ciclo vital individual e familiar da mulher com FM. O segundo artigo teve como objetivo compreender a repercussão do processo de desenvolvimento da dor no ciclo vital e familiar de mulheres com FM. Os resultados reafirmaram as repercussões da dor crônica, desde o diagnóstico até o tratamento, nos processos de desenvolvimento individual e familiar assim como na dimensão social e laboral, dificultando a sustentação e conciliação dos diferentes papéis desempenhadas pelas participantes. O terceiro artigo objetivou caracterizar os significados atribuídos à dor crônica e as estratégias de enfrentamento utilizadas por mulheres no contexto da FM. Evidenciou-se que as participantes compreendiam a doença como uma experiência desagradável e ao mesmo tempo, reconheciam os aspectos positivos do adoecer e, que alternavam tipos diferentes de coping frente à dor, prevalecendo estratégias de regulação emocional. Os significados demonstraram organizar o viver e conviver com a doença, direcionando as escolhas das estratégias de enfrentamento disponíveis. Enquanto resultado transversal aos três artigos, a dor crônica se sustentava num processo de invisibilidade, incompreensão, deslegitimação, resultando em estigmatização, tornando-se uma barreira no universo relacional das participantes e fonte de sofrimento psicológico. Conclui-se que essa dissertação avança no sentido de visibilizar e aprofundar a dimensão subjetiva presente na experiência da dor. Aponta-se sobre a necessidade de um cuidado integral e personalizado no contexto de FM, assim como sobre a emergência de políticas públicas voltadas para as intervenções psicossociais, na busca de melhor protagonismo e qualidade de vida das mulheres envolvidas. Abstract: Fibromyalgia (FM) is defined as a chronic pain syndrome, characterized by the presence of generalized musculoskeletal pain, with unclear etiopathogenesis, often associated with symptoms of fatigue, insomnia, functional disorders, cognitive changes and psychiatric conditions, occurring predominantly in women. Supported by the epistemological positioning of Complex Thinking, this qualitative research aimed to understand the meanings attributed to the experience of life and living with chronic pain from the perspective of adult women diagnosed with FM. Twelve women diagnosed with FM participated in the research. For data collection, a sociodemographic and clinical questionnaire and an in-depth semi-structured interview were used. The data content was organized and analyzed based on the coding process proposed by Grounded Theory (GT), with the help of the software for qualitative data Atlas.ti 23. The results were arranged in three articles, one theoretical article and two empirical ones. The first article was an integrative review, which sought to understand how scientific literature approaches the interface of FM in women and psychology. The results of the review highlighted the negative impact of FM on different areas of life and the need for greater production of qualitative studies that address the repercussions on the individual and family life cycle of women with FM. The second article aimed to understand the impact of the pain development process on the life and family cycle of women with FM. The results reaffirmed the repercussions of chronic pain, from diagnosis to treatment, on individual and family development processes as well as on the social and work dimensions, making it difficult to sustain and reconcile the different roles played by the participants. The third article aimed to characterize the meanings attributed to chronic pain and the coping strategies used by women in the context of FM. It was evident that the participants understood the disease as an unpleasant experience and, at the same time, recognized the positive aspects of becoming ill and that they alternated different types of coping with pain, with emotional regulation strategies prevailing. The meanings demonstrated to organize life and living with the disease, directing the choices of available coping strategies. As a transversal result across all three articles, chronic pain was sustained by a process of invisibility, misunderstanding, delegitimization, resulting in stigmatization, becoming a barrier in the participants' relational universe and a source of psychological suffering. It is concluded that this dissertation advances towards visualizing and deepening the subjective dimension present in the experience of pain. It highlights the need for comprehensive and personalized care in the context of FM, as well as the emergence of public policies aimed at psychosocial interventions, in the search for better protagonism and quality of life for the women involved. |
