Introdução: Pessoas com e sem perda auditiva apresentam desempenho prejudicado em situações ruidosas do dia a dia, uma vez que a compreensão de fala no ruído vem se tornando uma queixa constante. Dessa forma, os testes de reconhecimento de fala no ruído avaliam essa dificuldade simulando a escuta cotidiana. Contudo, há poucos testes padronizados na prática clínica, tanto para normo-ouvintes como para pessoas com perda auditiva. Objetivo: Analisar e comparar o desempenho de indivíduos normo-ouvintes no teste Reconhecimento Auditivo de Sentenças em Português (RASP) em relação ao sexo do locutor. Métodos: Foi aplicado o teste RASP em sujeitos normo-ouvintes adultos. Participaram do estudo 27 sujeitos, sendo 10 do sexo masculino e 17 do sexo feminino, com idade variando de 19 a 43 anos. Foi aplicado o questionário Scale of Auditory Behavior para excluir alterações de processamento auditivo central. O teste RASP é composto por 10 listas com 20 sentenças em cada lista, sendo possível o avaliador selecionar o sexo do locutor das listas de apresentação. O material de fala foi apresentado em campo livre com o paciente posicionado de frente, a um metro do alto-falante, a 0º azimute. Foram realizadas seis apresentações de listas, sendo a primeira na modalidade silêncio no qual foi analisado o Índice Percentual de Reconhecimento de Sentenças no Silêncio (IPRSS), a segunda na modalidade ruído adaptativo, na qual foi analisada a média da relação sinal/ruído (S/R), e a terceira na modalidade ruído fixo na relação S/R (0), no qual foi pesquisado o Índice Percentual de Reconhecimento de Sentenças no Ruído Fixo (IPRSR Fixo), sendo as três modalidades apresentadas tanto na voz masculina quanto na voz feminina. Foi analisado o desempenho conforme a ordem de apresentação, duração da lista, idade, sexo, escolaridade e pontuação no questionário SAB dos participantes. Resultados: A média de idade dos participantes foi de 27,4 anos. Verificou-se que a mediana do IPRSS foi de 100 % tanto usando as listas com o locutor masculino como feminino. A mediana da relação S/R foi -0,93 dB (DP -1.47 a -0,40) e 0,00 dB (-0,93 a 0,17) para o sexo masculino e sexo feminino respectivamente. Na condição ruído fixo na relação S/R (0), a porcentagem de acertos foi de 96% e 90% para o sexo masculino e feminino respectivamente. Em relação a duração das listas, verificou-se que houve diferença estatisticamente significante em todas as condições pesquisadas, sendo a duração menor com a locutora feminina. Verificou-se que não houve diferença estatisticamente significativa no desempenho dos sujeito em relação a ordem de apresentação do sexo do locutor das listas. Em relação à idade, verificou-se que houve correlação inversamente proporcional entre a porcentagem de acertos no IPRSR Fixo com a idade, ao usar listas com a locutora feminina. Quanto ao sexo dos participantes, verificou-se que as mulheres tiveram melhor desempenho no IPRSS ao utilizar listas com a locutora feminina. Já a escolaridade dos sujeitos e a pontuação no SAB, não tiveram associação com o desempenho no teste RASP. Conclusão: O sexo do locutor influenciou diretamente nos resultados do teste RASP. O desempenho dos indivíduos no IPRSS foi significativamente menor, a média da relação S/R a partir do ruído adaptativo foi mais elevada e o IPRSR Fixo foi significativamente menor quando os indivíduos foram expostos a listas gravadas com a locução feminina. A duração de apresentação das listas com locução feminina foi menor em todas as modalidades testadas. Quanto maior a idade dos sujeitos, pior o desempenho no IPRSR Fixo na locução feminina. Já em relação ao sexo dos participantes, as mulheres tiveram melhor desempenho no IPRSS do teste RASP quando utilizadas listas com locução feminina. Não houve influência no desempenho dos indivíduos em relação a ordem de apresentação das vozes, da escolaridade e na pontuação do SAB. O estudo forneceu evidências de um padrão de normalidade para o teste RASP, que surge como um importante instrumento de avaliação do reconhecimento de fala, demonstrando as condições reais de escuta da fala no silêncio e no ruído. Abstract: Introduction: People with and without hearing loss have impaired performance in everyday noisy situations, since speech comprehension in noise has become a constant complaint. Thus, speech recognition tests in noise assess this difficulty by simulating everyday listening. However, there are few standardized tests in clinical practice, both for normal-hearing individuals and for people with hearing loss. Purpose: To analyze and compare the performance of normal-hearing individuals in the Auditory Recognition of Sentences in Portuguese (RASP) test in relation to the gender of the speaker. Methods: The RASP test was applied to normal-hearing adult subjects. Twenty-seven subjects participated in the study, 10 males and 17 females, with ages ranging from 19 to 43 years. The Scale of Auditory Behavior questionnaire was applied to exclude central auditory processing disorders. The RASP test consists of 10 lists with 20 sentences in each list, and the evaluator can select the gender of the speaker from the presentation lists. The speech material was presented in a free field with the patient positioned facing the speaker, one meter from the speaker, at 0º azimuth. Six presentations of lists were performed, the first in silence mode, in which the Sentence Recognition Rate in Silence was analyzed; the second in adaptive noise mode, in which the average signal-to-noise (S/N) ratio was analyzed; and the third in fixed noise mode at the S/N ratio (0), in which the Sentence Recognition Rate in Fixed Noise was investigated; the three modalities were presented in both male and female voices. Performance was analyzed according to the order of presentation, list length, age, gender, education level, and score on the SAB questionnaire of the participants. Results: The mean age of the participants was 27.4 years. It was found that the median of the IPRSS was 100% using both the lists with the male and female announcer. The median of the mean S/N ratio was -0.93 dB (SD -1.47 to -0.40) and 0.00 dB (-0.93 to 0.17) for males and females, respectively. In the fixed noise condition at the S/N ratio (0), the percentage of correct answers was 96% and 90% for males and females, respectively. Regarding the duration of the lists, it was found that there was a statistically significant difference in all conditions studied, with the duration being shorter with the female announcer. It was found that there was no statistically significant difference in the performance of the subjects in relation to the order of presentation of the sex of the announcer of the lists. Regarding age, it was found that there was an inversely proportional correlation between the percentage of correct answers in the fixed noise condition and age, when using lists with a female speaker. Regarding the gender of the participants, it was found that women performed better in the IPRSS when using lists with a female speaker. The subjects' education level and the SAB score were not associated with performance in the RASP test. Conclusion: The gender of the speaker directly influenced the results of the RASP test. The performance of the individuals in the IPRSS was significantly lower, the average S/N ratio from the adaptive noise was higher and the fixed noise condition was significantly lower when the individuals were exposed to recorded lists with female voiceovers. The presentation duration of the lists with female voiceovers was shorter in all modalities tested. The older the subjects, the worse the performance in the fixed noise condition in the female voiceover. Regarding the gender of the participants, women had better performance in the IPRSS of the RASP test when using lists with female voiceovers. There was no influence on the performance of the individuals in relation to the order of presentation of the voices, education level or SAB score. The study provided evidence of a normality pattern for the RASP test, which emerges as an important instrument for assessing speech recognition, demonstrating the real conditions of listening to speech in silence and in noise. |
