Title: HIPERCOLESTEROLEMIA FAMILIAR: ANÁLISE DO ESPECTRO DE MUTAÇÕES DESCRITAS NO BRASIL

Abstract: A Hipercolesterolemia Familiar (HF) é uma condição genética autossômica dominante que causa elevação significativa do colesterol LDL (LDL-c) e aumenta o risco de doenças cardiovasculares precoces. Este trabalho teve como objetivo elencar as mutações já descritas em genes relacionados a HF na população brasileira, verificar seu provável efeito biológico através de ferramentas de predição, além de identificar e compreender os novos medicamentos utilizados para o seu tratamento. Foram encontrados 8 estudos e identificadas 98 mutações associadas à doença, distribuídas nos genes LDLR (88,8%), APOB (8,2%) e PCSK9 (3,1%). As variantes foram majoritariamente do tipo missense (70,4%), com predominância de alterações classificadas como patogênicas (61,2%) nas ferramentas de predição. O estudo também abordou cinco medicamentos para tratamento da HF, incluindo inibidores da PCSK9, inibidores da proteína de transferência de triglicerídeos microssomal (MTP) e oligonucleotídeos antisense, destacando sua eficácia (redução de aproximadamente 50% de LDL-c) e limitações, como reações adversas (especialmente hepáticas) e suas respectivas situações regulatórias no contexto brasileiro. Dentre os medicamentos citados, apenas dois estão disponíveis para comercialização no Brasil atualmente, o Evolocumabe e a Lomitapida, sendo que nenhum deles é disponibilizado pelo SUS. Os resultados reforçam a necessidade de novos estudos voltados à população brasileira, com o objetivo de compreender melhor o perfil genético da HF no país o que facilitaria o diagnóstico e tratamento adequado.Familial Hypercholesterolemia (FH) is an autosomal dominant genetic condition that causes a significant increase in LDL cholesterol (LDL-c) levels and raises the risk of early cardiovascular disease. This study aimed to list the mutations already described in genes related to FH in the Brazilian population, assess their probable biological effect using prediction tools, and identify and understand the new medications used for its treatment. Eight studies were identified, reporting 98 disease-associated mutations distributed among the LDLR (88.8%), APOB (8.2%), and PCSK9 (3.1%) genes. Most variants were of the missense type (70.4%), with a predominance of those classified as pathogenic (61.2%) by prediction tools. The study also addressed five medications for FH treatment, including PCSK9 inhibitors, microsomal triglyceride transfer protein (MTP) inhibitors, and antisense oligonucleotides, highlighting their efficacy (approximately 50% reduction in LDL-c) and limitations, such as adverse effects (especially hepatic) and their regulatory status in Brazil. Among the drugs discussed, only two - Evolocumab and Lomitapide - are currently available for sale in Brazil, and neither is provided by the public health system (SUS). The findings reinforce the need for further research focused on the Brazilian population to better understand the genetic profile of FH in the country, which would facilitate accurate diagnosis and appropriate treatment.
