| Title: | Terapêutica canabinóide no manejo da dor orofacial crônica: uma revisão sistemática |

O objetivo desta revisão sistemática foi investigar na literatura a eficácia da utilização de canabinoides, naturais ou sintéticos, no manejo da dor orofacial crônica. Realizou-se uma busca sistemática da literatura a fim de responder à seguinte pergunta: “Em pacientes com dor orofacial crônica, a utilização de canabinoides é eficaz no manejo da dor orofacial crônica?” A revisão seguiu as diretrizes do protocolo PRISMA, com a seleção dos estudos conduzida por dois revisores independentes. As buscas foram realizadas nas bases PubMed, Scopus, EMBASE, Cochrane, LILACS e Web of Science, bem como na literatura cinzenta. Inicialmente, foram identificados 1.430 estudos; após a aplicação dos critérios de elegibilidade, oito estudos foram incluídos na análise. Dos estudos selecionados, sete relataram efeitos positivos da utilização de canabinoides no manejo da dor orofacial crônica, abrangendo condições como disfunção temporomandibular muscular e articular, úlcera aftosa recorrente e síndrome da boca ardente. Os resultados destacaram propriedades analgésicas, anti-inflamatórias e miorrelaxantes, especialmente associadas ao canabidiol. Em contraste, a administração de palmitoiletanolamida em casos de neuropatia pós-herpética apresentou efeitos limitados. Em síntese, os canabinoides demonstraram potencial terapêutico no manejo da dor orofacial crônica, tanto de origem neuropática quanto nociceptiva. No entanto, devido a heterogeneidade dos estudos, a generalização dos resultados fica limitada. The objective of this systematic review was to investigate the available evidence regarding the use of cannabinoids, both natural and synthetic, in the management of chronic orofacial pain. A systematic literature search was conducted to answer the following question: In patients with chronic orofacial pain, is the use of cannabinoids effective in managing pain and the main symptoms of the condition, compared to conventional therapies or baseline values? The review followed the PRISMA protocol guidelines, with study selection conducted by two independent reviewers. Searches were carried out in the PubMed, Scopus, EMBASE, Cochrane, LILACS, and Web of Science databases, as well as in grey literature. Initially, 1,430 studies were identified; after applying eligibility criteria, eight studies were included in the analysis. Of the selected studies, seven reported positive effects of cannabinoids in managing chronic orofacial pain, including conditions such as muscular and articular temporomandibular dysfunction, recurrent aphthous ulcer, and burning mouth syndrome. The results highlighted analgesic, anti-inflammatory, and muscle-relaxant properties, particularly associated with cannabidiol. In contrast, the administration of palmitoylethanolamide in cases of post-herpetic neuralgia showed limited effects. In summary, cannabinoids demonstrated therapeutic potential in the management of chronic orofacial pain, whether of neuropathic or nociceptive origin. However, due to the heterogeneity of the studies, the generalizability of the results remains limited. |
