Abstract: Este artigo relata o caso clínico de um paciente de 53 anos, do sexo feminino, diagnosticado com linfoma de Burkitt (LB) com comprometimento do sistema nervoso central (SNC). O diagnóstico inicial foi realizado em 2021, instituído tratamento quimioterápico com o protocolo Hyper-CVAD em oito ciclos com administração intratecal. O quadro clínico envolvia comprometimento ósseo (mandíbula), SNC e presença de nódulo mamário. Em 2024, a paciente foi readmitida no Hospital Universitário com queixas de tontura, sonolência, náuseas, vômitos e episódios de diarreia; sem febre ou cefaleia. Diante do histórico oncológico e da gravidade do quadro, houve suspeita de recidiva do linfoma. Foi realizado biópsia cerebelar, com amostras de tecido enviadas para os laboratórios de imunofenotipagem e anatomopatologia do hospital. A citometria de fluxo revelou a presença de células linfoides B maduras com expressão de antígenos específicos, o que confirmou a recidiva do linfoma de Burkitt com acometimento do SNC. Esses resultados foram compatíveis àqueles observados na imuno-histoquímica. O esquema terapêutico de escolha foi de rituximabe, HDMTX e citarabina. Embora o diagnóstico precoce tenha possibilitado o tratamento adequado, o acometimento do SNC representou um fator de piora clínica e, consequentemente, no óbito da paciente. Apesar de a imunofenotipagem por citometria de fluxo não ser comumente aplicada à prática clínica para avaliação de tumores sólidos, sua capacidade de fornecer resultados com rapidez destaca a relevância desse método para o diagnóstico de neoplasias, especialmente nos casos de doenças com rápida progressão. A associação da imunofenotipagem por citometria de fluxo com a imuno-histoquímica pode sobrepor as limitações individuais de cada uma das metodologias e com isso ampliar a cobertura diagnóstica para tumores sólidos com resultados fidedignos, de forma mais rápida e eficiente, resultando em diagnósticos mais precoces e prognósticos mais acurados.This article reports the clinical case of a 53-year-old female patient diagnosed with Burkitt lymphoma (BL) with central nervous system (CNS) involvement. The initial diagnosis was made in 2021, and chemotherapy treatment was instituted with the Hyper-CVAD protocol in eight cycles with intrathecal administration. The clinical presentation bone involvement (mandible), CNS, and the presence of a breast lump. In 2024, the patient was readmitted to the University Hospital with complaints of dizziness, drowsiness, nausea, vomiting, episodes of diarrhea, without fever or headache. Given her oncological history and the severity of her condition, there was suspicion of lymphoma recurrence. A cerebellar biopsy was performed, with tissue samples sent to the hospital's immunophenotyping and anatomopathology laboratories. Flow cytometry revealed the presence of mature B lymphoid cells with expression of specific antigens, which confirmed the recurrence of Burkitt lymphoma with CNS involvement. These results were consistent with those observed in immunohistochemistry. The therapeutic regimen of choice was rituximab, HDMTX, and cytarabine. Although early diagnosis enabled appropriate treatment, CNS involvement represented a factor of clinical worsening and, consequently, the patient's death. Although immunophenotyping by flow cytometry is not commonly applied in clinical practice for the evaluation of solid tumors, its ability to provide results quickly highlights the relevance of this method for the diagnosis of neoplasms, especially in cases of rapidly progressing diseases, such as those affecting the CNS. The combination of immunophenotyping by flow cytometry with immunohistochemistry can overcome the individual limitations of each methodology and thereby expand diagnostic coverage for solid tumors with reliable results, more quickly and efficiently, resulting in earlier diagnoses and more accurate prognoses.
