Conhecimento, atitude e prática dos profissionais de saúde sobre os cuidados paliativos em uma unidade neonatal

Abstract

Introdução: Ao cuidar de recém-nascidos, a equipe de saúde é tomada por diversos sentimentos, alguns deles conflitantes, principalmente em relação a pacientes instáveis ou no fim da vida. Os cuidados paliativos são uma abordagem interdisciplinar, que visa atender o recém-nascido sem possibilidade de cura, bem como sua família, porém, ainda é um assunto que necessita de mais enfoque na neonatologia. Objetivo: Analisar o conhecimento, a atitude e a prática da equipe de saúde de uma unidade neonatal sobre cuidados paliativos. Método: O estudo foi desenvolvido em duas etapas, sendo a primeira uma revisão de escopo e a segunda um estudo avaliativo do tipo Inquérito Conhecimento, Atitude e Prática, de corte transversal e abordagem quantitativa. O contexto da pesquisa foi a Unidade Neonatal do Hospital Universitário da Universidade Federal de Santa Catarina. A coleta de dados da primeira etapa ocorreu no período de novembro de 2023 até fevereiro de 2024 nas fontes de dados The Scientific Electronic Library Online (Scielo), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Medical Literature Analysis and Retrieval System Online (MEDLINE) e Web of Science (Clarivate Analytics). A delimitação temporal dos estudos foi de 2018 a 2024. Foram selecionados 18 artigos com recomendações de Cuidados Paliativos Neonatais. A coleta de dados da segunda etapa foi feita em janeiro e fevereiro de 2024, através de questionário com perguntas articuladas acerca do tema abordado. Participaram desta etapa 66 profissionais da equipe de saúde que atuam na unidade. Os dados encontrados foram exportados para a Microsoft Excel e analisados pelo programa estatístico Stata/SE 13. O projeto de pesquisa foi avaliado e aprovado pelo Comitê de Ética em Pesquisa com Seres Humanos sob parecer consubstanciado nº 6.448.328. Resultados: As principais recomendações encontradas na revisão de escopo foram: melhorar a instrumentalização da equipe com capacitações diversas, desenvolver habilidades de comunicação, estratégias voltadas para o atendimento perinatal e para os familiares, e definição de diretrizes institucionais. Em relação ao conhecimento, atitude e prática foi possível perceber que a equipe da unidade investigada possui em geral, um score de conhecimento (8,0) e atitude (7,0) altos, porém um score de prática (3,0) baixo, e houve diferenças estatísticas em relação a formação e possuir pós-graduação, no score de conhecimento e de prática se recebeu ou não informações sobre Cuidados Paliativos durante a graduação. Conclusão: A partir da avaliação dos scores, é possível perceber que as práticas sobre cuidados paliativos precisam ser aprimoradas. É necessário investigar o conhecimento dos profissionais acerca deste tema mais profundamente, buscando localizar possíveis lacunas que o presente estudo não tenha alcançado, a fim de melhorar as práticas realizadas na unidade. São necessários maiores investimentos nesta temática, desde a formação dos profissionais até o envolvimentos dos gestores da instituições de saúde.

Abstract: Introduction: When caring for newborns, the healthcare team is overwhelmed by various feelings, some of them conflicting, especially in relation to unstable patients or those at the end of their lives. Palliative care is an interdisciplinary approach that aims to assist the newborn without the possibility of a cure, as well as their family, but it is still a subject that needs more focus in neonatology. Objectives: To analyze the knowledge, attitude, and practice of the health team of a neonatal unit on palliative care. Method: The study was carried out in two stages, the first being a scoping review and the second a cross-sectional, quantitative evaluation study of the Knowledge, Attitude and Practice Survey type. The research context was the Neonatal Unit of the University Hospital of the Federal University of Santa Catarina. Data collection for the first stage took place from November 2023 to February 2024 using the Scientific Electronic Library Online (Scielo), Latin American and Caribbean Health Sciences Literature (LILACS), Medical Literature Analysis and Retrieval System Online (MEDLINE) and Web of Science (Clarivate Analytics) data sources. The time frame of the studies was from 2018 to 2024. Eighteen articles with recommendations on Neonatal Palliative Care were selected. The second stage of data collection took place in January and February 2024, using a questionnaire with articulated questions on the subject. Sixty-six professionals from the health team working in the unit took part in this stage. The data was exported to Microsoft Excel and analyzed using the program Stata/SE 13. The research project was assessed and approved by the Human Research Ethics Committee under substantiated opinion no. 6.448.328. Findings: The main recommendations found in the scoping review were improving the skills of the team with various training courses, developing communication skills, strategies aimed at perinatal care and family members, and defining institutional guidelines. In relation to knowledge, attitude, and practice, it was possible to see that the staff of the unit investigated generally had a high knowledge score (8.0) and attitude score (7.0), but a low practice score (3.0), and there were statistical differences in relation to training, and having a postgraduate degree, in the knowledge and practice score of whether or not they received information about Palliative Care during their undergraduate studies. Conclusion: From the evaluation of the scores, it is possible to see that palliative care practices need to be improved. It is necessary to investigate in greater depth the knowledge of professionals on the topic, seeking to locate possible gaps that this study did not reach, in order to improve the practices carried out in the unit. These thematic investments are necessary, from the training of top professionals to the involvement of managers of health institutions.
