Infiltração de células dendríticas em lesões centrais de células gigantes: correlação com agressividade e recorrência

Abstract

A lesão central de células gigantes (LCCG) consiste em uma lesão com características osteolíticas que ocorre nos maxilares. Por possuir um comportamento clínico variado, as LCCGs podem ser classificadas em agressivas ou não agressivas e, devido ao fato das duas manifestações serem histologicamente semelhantes, a diferenciação se torna complexa e leva em consideração apenas as características clínicas e radiográficas da lesão. As células dendríticas (CDs) são componentes do sistema imune e responsáveis por vigiar os tecidos frente a possíveis ameaças. Considerando sua importância para a defesa do organismo e a presença na literatura de estudos vinculando a depleção de CDs na agressividade de tumores em maxilares, este trabalho busca investigar a relação entre a infiltração de subpopulações de CDs e a agressividade das LCCGs. Para realização desta pesquisa, foram coletadas 62 amostras de LCCGs vinculadas ao Biobanco do Laboratório de Patologia Bucal da Universidade Federal de Santa Catarina (LPB-UFSC), com o intuito de obter informações clínicas e histopatológicas dos casos. A partir das informações clínicas foi criado um banco de dados e as lâminas histológicas foram submetidas a reações imuno-histoquímicas com os marcadores para CDs incluindo anti-CD1a, anti-CD207 e anti-HLA-DR. Testes paramétricos e não-paramétricos foram aplicados com o software SPSS, na comparação da quantificação de CDs e os parâmetros clínico-patológicos das LCCGs. As LCCGs recorrentes tiveram menor densidade de CD CD207+ quando comparadas com as lesões não-recorrentes (p = 0,038). No entanto, não houve diferença entre as lesões recorrentes e não-recorrentes para os marcadores CD1a e HLA-DR (p > 0,109). Nossos achados sugerem que a depleção de subpopulações de CDs com baixo nível de maturação em LCCGs pode ser indicativo de maior agressividade e taxa de recorrência em LCCGs.

Central giant cell granuloma (CGCG) consists of an osteolytic lesion that occurs in the jaw. As they have a varied clinical behavior, CGCGs can be classified as aggressive or non-aggressive and, due to the fact that both manifestations are histologically similar, differentiation becomes complex and only takes into account the clinical and radiographic characteristics of the lesion. Dendritic cells (DCs) are components of the immune system and are responsible for monitoring tissues against possible threats. Considering its importance for the body's defense and the presence in the literature of studies linking the depletion of DCs to the aggressiveness of tumors in the jaw, this work seeks to investigate the relationship between the infiltration of subpopulations of DCs and the aggressiveness of CGCGs. For this research, were collected 62 samples of CGCGs from the Biobank of the Oral Pathology Laboratory of the Federal University of Santa Catarina (LPB-UFSC), with the aim of obtaining clinical and histopathological information on the cases. From the clinical information, a database was created and the histological slides were subjected to immunohistochemical reactions with markers for DCs including anti-CD1a, anti-CD207 and anti-HLA-DR. Parametric and non-parametric tests were applied with the SPSS software, to compare the quantification of DCs and the clinicopathological parameters of CGCGs. Recurrent CGCGs had lower CD207+ DC density when compared to non-recurrent lesions (p = 0.038). However, there was no difference between recurrent and non-recurrent lesions for CD1a and HLA-DR markers (p > 0.109). Our findings suggest that the depletion of DC subpopulations with a low level of maturation in CGCGs may be indicative of greater aggressiveness and recurrence rate in CGCGs.
