Leucemia mieloide aguda com fusão CBFB::MYH11 e presença de mastócitos aberrantes: um relato de caso

Abstract

Este trabalho tem como objetivo relatar um caso clínico de um paciente com 51 anos, do sexo masculino, com diagnóstico de leucemia mieloide aguda (LMA) com fusão CBFB::MYH11, mutação FLT3-D835 e presença de mastócitos aberrantes. O paciente foi admitido com fadiga, sudorese noturna, perda ponderal e episódios de perda de consciência. Os exames iniciais revelaram anemia, leucocitose com predomínio de blastos e trombocitopenia, sugerindo leucemia aguda. A imunofenotipagem da medula óssea confirmou a presença de blastos comprometidos com a linhagem mieloide e monocítica e as análises genéticas e moleculares adicionais identificaram o cariótipo 46,XY,inv(16)(p13q22) e a fusão CBFB::MYH11, concluindo o diagnóstico de LMA com fusão CBFB:MYH11. Adicionalmente, o paciente apresentou a mutação FLT3-D835 analisada por biologia molecular. O paciente iniciou quimioterapia de indução ARA-C + Daunorrubicina. Um mês após o início do tratamento, a imunofenotipagem mostrou uma redução significativa dos blastos, mas identificou mastócitos aberrantes. Embora esses resultados sugeriram mastocitose sistêmica, o paciente não cumpriu todos os critérios diagnósticos estabelecidos pela Organização Mundial da Saúde (OMS), apontando para uma LMA associada a mastocitose. Seis meses após o diagnóstico inicial, o paciente não apresentou blastos leucêmicos, mas houve presença de células monocíticas imaturas e mastócitos. Este caso ressalta a importância de um diagnóstico preciso e acompanhamento multidisciplinar em neoplasias hematológicas, utilizando técnicas de imunofenotipagem e biologia molecular para o diagnóstico, prognóstico e monitoramento da resposta ao tratamento. Para casos como o relatado neste estudo, o distúrbio hematológico inicial deve ser tratado e o manejo em relação a presença dos mastócitos deve incluir monitoramento de infiltrados, sintomas e observação de critérios que estabeleçam a progressão da doença.

This article report a clinical case of a 51-year-old male patient diagnosed with acute myeloid leukemia (AML) with CBFB::MYH11 fusion, FLT3-D835 mutation and presence of aberrant mast cells. The patient was admitted with fatigue, night sweats, weight loss, and episodes of loss of consciousness. Initial tests revealed anemia, leukocytosis with a predominance of blasts, and thrombocytopenia, suggesting acute leukemia. Bone marrow immunophenotyping confirmed the presence of blasts committed to the myeloid and monocytic lineages. Additional genetic and molecular analyses identified the karyotype 46,XY,inv(16)(p13q22) and the CBFB::MYH11 fusion, concluding the diagnosis of AML with CBFB fusion. Additionally, the patient presented the FLT3-D835 mutation analyzed by molecular biology. The patient started induction chemotherapy with ARA-C + Daunorubicin. One month after starting the treatment, immunophenotyping showed a significant reduction in blasts but identified aberrant mast cells. Although these results suggested systemic mastocytosis, the patient did not meet all the diagnostic criteria established by the World Health Organization (WHO), pointing to AML associated with mastocytosis. Six months after the initial diagnosis, the patient did not present leukemic blasts, but there was the presence of immature monocytic cells and mast cells. This case highlights the importance of precise diagnosis and multidisciplinary follow-up in hematological neoplasms, using immunophenotyping and molecular biology techniques for diagnosis, prognosis, and treatment response monitoring. For cases like the one reported in this study, the initial hematologic disorder should be treated, and the management regarding the presence of mast cells should include monitoring of infiltrates, symptoms, and observation of criteria that establish disease progression.
