A Medicina de Famílis e Comunidade na Reforma Sanitária e na Disputa do SUS: uma revisão narrativa

Abstract

Introdução: A Medicina de Família e Comunidade é uma classe profissional que exerce influência cada vez maior sobre os rumos da saúde no Brasil. Historicamente, seu estabelecimento teve grande impacto na estruturação da APS no SUS, e nas transformações epistemológicas pelas quais a prática clínica e o conceito de saúde passaram, movimentos esses que fazem parte dos objetivos programáticos da Reforma Sanitária Brasileira (RSB). Sendo assim, o conhecimento dos rumos da MFC enquanto classe é importante para conhecermos os rumos da Reforma Sanitária e do SUS. Objetivos: Fazer uma breve revisão sobre a MFC, a RSB e a relação entre elas, além de apresentar alguns discursos internos à MFC de hoje. Métodos: Uma revisão narrativa cuja bibliografia de atenção foi selecionada a partir de publicações relevantes e de alto impacto no campo da Saúde Coletiva e da MFC, levando como critérios de seleção a conveniência, a notoriedade e buscas manuais feitas pelo autor. Discussão: As tendências discursivas dentro da MFC apresentam projetos destoantes entre si e destoantes com os rumos históricos da classe, tornando a continuidade da consonância entre a MFC e os objetivos da RSB incerta. Conclusão: É fato que o estabelecimento e os rumos até aqui da MFC no Brasil foram benéficos para os objetivos da RSB, porém essa continuidade não é garantida.

Introduction: The Family and Community Medicine (MFC) is a class that has a growing influence in the directions of health in Brazil. Historically, its establishment had great impact on the structuring of the Primary Healthcare (PHC) of the brazilian Unified National Health System (SUS), and on the epistemological changes that the clinical practice and the health conceptualization had passen through, a movement that is part of the objectives of the Brazilian Sanitary Reform (RSB). Therefore, the knowledge of the directions of MFC as a class is important for knowing the directions of the Sanitary Reform as a hole and of the SUS. Objectives: Make a brief review about MFC, RSB and the relationship between them, besides a presentation of some internal speeches on today’s MFC. Methods: A narrative review whose attention biography was selected from relevant and high impact publications on Collective Health and MFC fields, the selection criteria were convenience, notability and manual searches made by the author. Discussion: The narrative tendencies inside MFC have projects that are discordant between them and with the historical paths of the class, making the continuity of the consonance between MFC and the objectives of the RSB uncertain. Conclusion: It is a fact that the establishment and the historical paths until here of MFC in Brazil were beneficial to the objectives of RSB, but its continuity is not assured.
