Farmacoterapia no tratamento da disfunção temporomandibular : revisão da literatura

Abstract

A disfunção temporomandibular (DTM) é um termo genérico que inclui a articulação temporomandibular (ATM), os músculos mastigatórios e suas estruturas associadas. As DTMs são caracterizadas por dor, sons articulares e movimento mandibular restrito, sendo a dor um motivo comum para a procura por atendimento odontológico. Apesar de ser frequente, o diagnóstico e o tratamento das DTMs ainda é um desafio para o cirurgião dentista, por ser uma condição complexa e de etiologia multifatorial. É essencial que os cirurgiões dentistas sejam capazes de diagnosticar corretamente e implementar medidas de tratamento efetivo para essa condição. Dentro deste contexto muitas terapias têm sido propostas para o manejo das DTMs, incluindo o uso de diferentes medicamentos. Este estudo teve como objetivo realizar uma revisão narrativa da literatura sobre os tratamentos farmacológicos utilizados no tratamento de disfunção temporomandibular. As fontes de busca foram livros textos e artigos eletrônicos de acesso livre nas bases de dados: COCHRANE, LILACS, PUBMED, SCIELO e Google Scholar. A pesquisa foi limitada aos artigos publicados na língua portuguesa ou inglesa, com abrangência temporal entre os anos de 1987 e 2022. As buscas foram realizadas no período de março de 2021 até agosto de 2022, sendo utilizados 92 artigos e 5 livros textos para a confecção deste projeto. Os critérios de inclusão da literatura foram ser escrito em português ou inglês, e ter o conteúdo relacionado com o objetivo da pesquisa. Após uma ampla busca por informações, conclui-se que a DTM é uma condição que acomete parte significativa da população e que sua terapia pode ser realizada de diversas formas, sendo a medicamentosa uma delas. No entanto, o desconhecimento dos mecanismos subjacentes à dor associada a esta patologia e o fato de os estudos realizados até agora utilizarem critérios de seleção de pacientes altamente díspares, os resultados sobre a eficácia dos diferentes medicamentos comumente utilizados são inconclusivos, impossibilitando o desenvolvimento de um protocolo adequado e amplamente aceito, seja para o atendimento de urgência ou mesmo para o tratamento visando a cura.

Temporomandibular disorder (TMD) is a generic term that includes the temporomandibular joint (TMJ), the masticatory muscles and their associated structures. TMDs are characterized by pain, joint sounds and restricted jaw movement, with pain being a common reason for seeking dental care. Although the diagnosis and treatment of TMDs are frequent, it is still a challenge for the dental surgeon, as it is a complex condition with a multifactorial etiology. It is essential that dental surgeons are able to correctly diagnose and implement effective treatment measures for this condition. Within this context, many therapies have been proposed for the management of TMDs, including the use of different medications. This study aimed to carry out a narrative review of the literature on the pharmacological treatments used in the treatment of temporomandibular disorders. The search sources were textbooks and open access electronic articles in databases such as COCHRANE, LILACS, PUBMED, SCIELO and Google Scholar. The search was limited to articles published in Portuguese or English, with a temporal scope between the years 1987 and 2022. The searches were carried out from March 2021 to August 2022, using 92 articles and 5 textbooks for the preparation of this project. After an extensive search for information, it was concluded that TMD is a condition that affects a significant portion of the population and that its therapy can be performed in different ways, with medication being one of them. However, the lack of knowledge on the mechanisms underlying the pain associated with this pathology and the fact that the studies carried out so far use highly disparate patient selection criteria, the results on the effectiveness of the different drugs commonly used are inconclusive, making it impossible to develop an adequate and widely accepted protocol, whether for urgent care or even for treatment aimed at cure.
