Desenvolvimento de um fluxograma para o atendimento a mulheres em situação de violência sexual

Abstract

Estudo com objetivo de elaborar um fluxograma de articulação dos serviços intersetoriais de atendimento à mulher em situação de violência sexual, em um município do norte do Rio Grande do Sul. Delineamento metodológico realizado em quatro etapas: abordagem qualitativa, Revisão Integrativa da Literatura (RIL), estudo descritivo (mapeamento) e validação de conteúdo. Na etapa qualitativa, estudo com membros da equipe multiprofissional (30 profissionais) que atuam nos serviços de atendimento à mulher em situação de violência sexual. Coleta de dados por meio da entrevista semiestruturada. Realizada análise temática e utilizouse o software Atlas.ti® 7.5.6 para auxiliar na organização dos dados. A RIL, segunda etapa, foi realizada em quatro bases de dados e em uma biblioteca digital, amostra final composta por 34 estudos. Na terceira etapa, ocorreu o mapeamento e georreferenciamento dos serviços de atendimento à mulher em situação de violência sexual no município. Para o georreferenciamento dos serviços foi utilizado o Software ArcGIS® 10.1. Na última etapa, a validação de conteúdo ocorreu por meio da técnica Delphi, a amostra constituiu em 14 juízes que responderam o formulário. Análise estatística realizada por meio do Índice de Validade de Conteúdo e Kappa. Os resultados foram organizados conforme as etapas e um capítulo acerca da elaboração do fluxograma. Na etapa qualitativa, foram identificadas as potencialidades e fragilidades do atendimento intersetorial à mulher em situação de violência sexual. Sinalizada a necessidade da articulação dos serviços e qualificação dos profissionais. A RIL evidenciou que há potencialidades do atendimento à mulher em situação de violência sexual, mas sobressaem fragilidades, como incipiente e inexistente articulação da rede de atendimento intersetorial, gerando demandas de construção, articulação e sustentabilidade da rede. No mapeamento dos serviços, com o georreferenciamento e a produção dos mapas, obteve-se a identificação dos espaços existentes no território, podendo projetar possibilidades de fluxo e por consequência melhorar a articulação entre os serviços. Na elaboração e validação do fluxograma foi descrito os passos da adapatação que ocorrreu a partir do: Fluxogramas de Atendimento às Pessoas em Situação de Violência Sexual nos Serviços Especializados de Referência da Saúde; documentos disponíveis online: Norma técnica: Prevenção e tratamento dos agravos resultantes da violência sexual contra mulheres e adolescentes; Rede de Enfrentamento à Violência contra as Mulheres; resultados da etapa qualitativa, da RIL e do mapeamento. Considerando o substantivo conjunto de informações coletadas, pode-se elaborar e validar o Fluxograma para Atendimento à Mulher em Situação de Violência Sexual. Este fluxograma possibilita a integralidade no atendimento por meio da articulação dos serviços intersetoriais e está inserido na proposta municipal e regional para o atendimento à mulher em situação de violência sexual. Conclusões: o fluxograma é parte essencial na articulação e passou a constituir um referencial para o município. Relevância para a área da enfermagem, da saúde e para a sociedade: contribui para o processo de trabalho em enfermagem e equipe interdisciplinar e transdisciplinar, direcionando condutas resolutivas na área da saúde e no atendimento à mulher, bem como, promoção da articulação intersetorial, na região em estudo.

Abstract: Study with the objective of elaborating a flowchart for the articulation of intersectoral services for women in situations of sexual violence, in a municipality in the north of Rio Grande do Sul. Methodological design carried out in four stages: qualitative approach, Integrative Literature Review (RIL), descriptive study (mapping) and content validation. In the qualitative stage, I study with members of the multiprofessional team (30 professionals) who work in the services of assistance to women in situations of sexual violence. Data collection through semi-structured interview. Thematic analysis was performed and the Atlas.ti® 7.5.6 software was used to help organize the data. The second stage of the RIL was carried out in four databases and in a digital library, a final sample consisting of 34 studies. In the third stage, the mapping and georeferencing of services for women in situations of sexual violence occurred in the municipality. For the georeferencing of services, the ArcGIS® 10.1 Software was used. In the last step, the content validation occurred using the Delphi technique, the sample consisted of 14 judges who answered the form. Statistical analysis were performed using the Content Validity Index and Kappa. The results were organized according to the steps and a chapter on the elaboration of the flowchart. In the qualitative stage, the strengths and weaknesses of intersectoral care for women in situations of sexual violence were identified. The need for the articulation of services and qualification of professionals signaled. The RIL showed that there are potentialities for assisting women in situations of sexual violence, but weaknesses stand out, such as the incipient and inexistent articulation of the intersectoral care network, generating demands for the construction, articulation and sustainability of the network. In the mapping of services, with the georeferencing and the production of the maps, it was possible to identify the existing spaces in the territory, being able to project flow possibilities and, consequently, improve the articulation between the services. In the elaboration and validation of the flowchart, the steps of the adaptation that occurred from the: Flowcharts of Assistance to People in Situations of Sexual Violence in the Specialized Health Reference Services were described; documents available online: Technical standard: Prevention and treatment of injuries resulting from sexual violence against women and adolescents; Network to Combat Violence against Women; results of the qualitative stage, the RIL and the mapping. Considering the substantive set of information collected, it is possible to elaborate and validate the Flowchart for Assistance to Women in Situations of Sexual Violence. This flowchart allows comprehensive care through the articulation of intersectoral services and is inserted in the municipal and regional proposal for the care of women in situations of sexual violence. Conclusions: the flowchart is an essential part of the articulation and has become a reference for the municipality. Relevance for the area of nursing, health and society: it contributes to the nursing work process and interdisciplinary and transdisciplinary team, directing resolutive conduct in the area of health and care for women, as well as promoting intersectoral articulation in the study region.
