Saúde mental, trabalho e atuação em serviços de saúde do trabalhador: concepções e intervenções do psicólogo

Abstract

Os serviços públicos em Saúde do Trabalhador são espaços de atuação da Psicologia. Um dos principais serviços da Rede Nacional de Atenção Integral à Saúde do Trabalhador (RENAST) é o Centro de Referência em Saúde do Trabalhador (CEREST), constituído, entre outros profissionais, por psicólogos, cuja atuação tem sido pouca analisada e questionada em âmbito científico. Compreendendo este cenário de recente imersão do psicólogo e também de lacunas quanto a atuação, a presente dissertação teve como objetivo compreender as intervenções de psicólogos que atuam em CEREST e suas concepções sobre saúde, saúde mental e trabalho. Realizou-se um mapeamento nacional e concomitantemente foi enviado um questionário on-line que obteve 56 respostas, número que representa 50,0% da população, e que teve representação de todas as macrorregiões do país. Foram entrevistados sete participantes que se dispuseram a este segundo momento. Ambos os instrumentos foram criados pela pesquisadora e testados por profissionais da área. O período de coleta foi o ano de 2018. Os principais resultados encontrados foram: a maior parte da amostra (87,5%) é composta por mulheres, o que é coerente com o fato da Psicologia ser uma profissão majoritariamente feminina no Brasil. Foi identificado que as atuações predominantes circundam entre educação e vigilância e as demais seguem a heterogeneidade da gestão de cada CEREST, que inclusive, possui ações e percepções diversificadas entre educação e vigilância, o que é benéfico ao possibilitar adequações do serviço à cada realidade abrangida. Os pontos de convergência são: a maior parte dos psicólogos realizam as atividades centrais atribuídas aos CEREST e 100% atuam em conjunto com mais profissionais, sobretudo, com enfermeiros (92,8%). Há diversas particularidades na atuação, que podem ser explicadas por existir poucos guias e instrumentos para psicólogos em CEREST, pois há mais orientações direcionadas ao serviço, o que auxilia o psicólogo a realizar ações mais generalistas e poucas exclusivas de sua área. Este caráter generalista nas ações também foi explicado como característica das políticas públicas que os orientam, porém, pode interferir negativamente na identidade profissional do psicólogo. Esta questão também foi apontada como uma das dificuldades para a atuação, junto com os escassos recursos financeiro e apoio institucional. Identificou-se que o conhecimento dos psicólogos sobre Saúde do Trabalhador é baixo e em alguns casos até nulo e uma das explicações refere- se às grades curriculares abordarem pouco o tema, conforme exposto por 30% dos participantes. Isto é bastante grave e se mostra urgente a necessidade de maior balizamento de conhecimentos sobre saúde do trabalhador para futuros psicólogos que possam atuar em CEREST. Sobre as concepções dos participantes sobre Saúde, foi verificado que são similares, remetendo principalmente a bem-estar e como a somatória de vários fatores, o que se aproxima da definição preconizada pelo SUS, política ou sistema? no qual eles estão inseridos. Sobre Saúde Mental e Trabalho (SMT) a maior parte demonstrou adotar perspectivas similares à Psicodinâmica do Trabalho e à teoria de Subjetividade, e relataram que é fundamental realizar educação e vigilância, o que novamente demonstra coerência com o que de fato realizam. Sobre a RENAST, a maioria das opiniões que precisa ser articuladora mas tem muitas dificuldades a serem enfrentadas e as principais habilidades dos psicólogos em RENAST devem ser diálogo, articulação, criação de parcerias, criatividade e realização de vigilância e prevenção. Este estudo concluiu que as principais ações de psicólogos em CEREST são as que caracterizam os serviços, as concepções são um tanto similares, porém, em vários casos distintas do que seria coerente com o que o SUS e a RENAST preconizam. Revela-se de importância ampliar o número de estudos sobre o a atuação e concepções sobre psicólogos em CEREST.

Abstract: Public services in Worker's Health are spaces of action of Psychology. One of the main services of the National Network of Integral Attention to Workers' Health (RENAST) is the Occupational Health Reference Center (CEREST), made up, among other professionals, of psychologists whose work has been little analyzed and questioned in scientific terms . Understanding this scenario of recent immersion of the psychologist and also of shortcomings in acting, this dissertation aimed to understand the interventions of psychologists who work in CEREST and its conceptions on health, mental health and work. A national mapping was carried out, and an online questionnaire was sent, which received 56 responses, representing 50.0% of the population, and representing all macro regions of the country. Seven participants were interviewed who were willing to attend this second moment. Both instruments were created by the researcher and tested by professionals in the field. The collection period was the year 2018. The main results were: the majority of the sample (87.5%) is composed of women, which is consistent with the fact that Psychology is a predominantly female profession in Brazil. It was identified that the predominant activities are between education and surveillance, and the others follow the heterogeneity of the management of each CEREST, which also has diverse actions and perceptions between education and vigilance, which is beneficial in enabling service adaptations to each reality covered. The points of convergence are: most psychologists perform the central activities assigned to CEREST and 100% work together with more professionals, especially nurses (92.8%). There are several particularities in the performance, which can be explained by the fact that there are few guides and instruments for psychologists in CEREST, since there are more guidelines directed to the service, which helps the psychologist to perform more general actions and few exclusive ones in his area. This generalist character in the actions was also explained as characteristic of the public policies that guide them, however, it can interfere negatively in the professional identity of the psychologist. This issue was also pointed out as one of the difficulties for the operation, together with the scarce financial resources and institutional support. It was identified that the knowledge of the psychologists on Worker's Health is low and in some cases even null and one of the explanations refers to the curricular curricula approaching the subject little, as exposed by 30% of the participants. This is quite serious and it is urgent to see more need for more knowledge about worker health for future psychologists who can work at CEREST. Regarding the participants' conceptions about Health, it was verified that they are similar, referring mainly to well-being and as the sum of several factors, what is close to the definition advocated by the SUS, policy or system? in which they are inserted. Most of Mental Health and Work (SMT) have demonstrated similar perspectives to Work Psychodynamics and Subjectivity theory, and have reported that it is fundamental to carry out education and surveillance, which again shows consistency with what they actually do. About RENAST, most of the opinions that need to be articulated but have many difficulties to be faced and the main skills of psychologists at RENAST should be dialogue, articulation, partnership building, creativity and surveillance and prevention. This study concluded that the main actions of psychologists in CEREST are those that characterize the services, the conceptions are somewhat similar, however, in several different cases than would be consistent with what SUS and RENAST advocate. It is of importance to expand the number of studies on the performance and conceptions on psychologists in CEREST.
