Organização dos conselhos locais de saúde

Abstract

Os Conselhos Locais de Saúde (CLS) são órgãos colegiados, de caráter permanente, instalados junto a Unidade Básica de Saúde. O objetivo da pesquisa foi caracterizar a organização dos CLS no sistema de saúde brasileiro. Trata-se de um estudo descritivo-exploratório, com abordagem qualitativa. Inicialmente realizou-se uma revisão bibliográfica com finalidade de compreender as funções dos conselhos locais de saúde, conselhos de educação e conselhos de alimentação/ nutrição escolar. Posteriormente, uma análise documental, identificando as possibilidades de organização dos CLS existentes. Por fim, entrevistas semiestruturadas com especialistas no tema e análise temática, para compreender os pontos considerados essenciais para a organização dos CLS. Na revisão, foram incluídos dezenove artigos sobre CLS e vinte e um de educação. Não houve artigos selecionados no setor de nutrição e alimentação escolar. Os artigos revisados apresentaram maior número de dificuldades do que aspectos facilitadores. Os motivos apresentados são: descrédito pela gestão e pela comunidade sobre as discussões do conselho, desconhecimento da existência dos conselhos, falta de resolutividade das demandas, infraestrutura insuficiente, capacitação dos conselheiros e dificuldade de diálogo com instâncias do governo. Os locais com experiências exitosas têm em comum a busca de lideranças locais, realização de fóruns e reuniões abertas à comunidade e articulação política com o executivo. Na análise documental, foram incluídos dez documentos governamentais da esfera federal, que apoiam a descentralização do controle social e os CLS, porém não apresentam diretrizes orientadoras e definições que os situem no desenho do sistema de saúde. No âmbito municipal, oito documentos demonstraram clareza de definições do papel do CLS. Na análise temática apresentam-se três temas: Participação da Comunidade, onde se recomenda que profissionais e gestores conheçam e interajam com a comunidade, de forma que o CLS seja um espaço de diálogo, escuta e acolhimento; Formação, que demonstra a importância da qualificação da participação, utilizando-se do espaço do CLS para promover essas atividades; e Organização dos CLS, que destaca a necessidade de definição de competências do CLS, e a melhoria de aspectos operacionais como espaço físico, financiamento, construção de pautas, registro das reuniões, monitoramento dos encaminhamentos e articulação com o conselho municipal de saúde. O diálogo com a comunidade foi visto como a melhor maneira de decidir questões como as formas de comunicação, escolha de conselheiros, periodicidade, horários e definição de pautas das reuniões. Sugere-se que sejam realizadas orientações a nível nacional, ofertando suporte teórico para facilitar o entendimento de papéis dos CLS, limites de atuação e auxiliar na sua organização.

Abstract: The local Health Councils (LHC) are permanent collegiate organs, which are usually installed in the basic health unit. The aim of the research was to characterize the LHC organization in the brazilian health system. This is a descriptive-exploratory study with a qualitative approach. Initially, a bibliographic review was conducted with the purpose of understanding the functions of local health councils, education councils and school feeding/nutrition councils. Subsequently, a documentary analysis identifying the organization possibilities of the LHC that are working. Finally, a qualitative research, through semi-structured interviews with specialists in the theme and thematic analysis, to understand the points considered essential for the organization of LHC. In the review, nineteen articles on LHC and twenty-one education were included. There were no articles selected in the nutrition and school feeding sector. The articles reviewed presented a greater number of difficulties than facilitating aspects. The reasons given are: discredity of the council discussions by the management and the community, unfamiliarity with the existence of the councils, lack of resolution of demands, lack of infrastructure, qualification of counselors and difficulty of dialogue with government agencies. Locals with successful experiences have in common the search for local leaderships, the holding of forums and meetings open to the community, and political articulation with the executive. In the documentary analysis, ten governmental documents were included at the national level, which demonstrate support for decentralization of social control and LHC, but do not have guiding directives and definitions that place them in the design of the health system. At the municipal level, eight LHC documents were included, demonstrating greater clarity of definitions at this level. The thematic analysis presents three themes: community participation, which recommends that professionals and managers know and interact with the community, so that the LHC is a space for dialogue, listening and welcoming. Formation, which places the need for training and qualification of participation, using the space of the LHC to promote these activities; and LHC organization, which highlights the need to define LHC competencies and roles, and improve operational aspects such as physical space, financing, construction of schedules, registration of meetings, monitoring of referrals and articulation with the municipal health council. Dialogue with the community was seen as the best way to decide issues such as communication, choosing of counselors, periodicity, hours and meeting matters. It is suggested that guidance be developed at national level, offering theoretical support to facilitate the understanding of LHC roles, limits of actuation and help in their organization.
