Humanizando, acolhendo, (trans)formando: produção de sentidos sobre a assistência transespecífica em saúde

Abstract

Esta tese debruça-se na análise dos processos de produção de sentidos em torno da assistência transespecífica em saúde (ATS), de sorte a questionar e dar visibilidade à complexidade das formas pelas quais a transcidadania deve ser garantida. Buscou-se dialogar com as experiências das pessoas usuárias, médicos e funcionários da SMS vinculados ao serviço-atendimento de Florianópolis, assim como, com o marco legal que orienta o Processo Transexualizador no SUS e outras políticas/programas que servem de referência às noções de ATS com que se opera atualmente no campo da saúde no Brasil. Foram realizadas 11 entrevistas e o conceito de problematização recursiva proposto por Fernández (2007) foi utilizado como operador metodológico para a produção de informações, escolha que implica no âmbito metodológico a elaboração de campos de problemas. A análise das informações produzidas no campo permitiu uma leitura aprofundada e trans-situada das demandas que as pessoas usuárias trazem no encontro com esses serviços-atendimentos e suas implicações frente à discussão dos sentidos aos que se atrela a ATS. Discutiu-se a ATS como forma de concretização de direitos, ainda que parciais, e suas particularidades no contexto da APS e a Estratégia de Saúde da Família e Comunidade. Problematizou-se a ATS como espaço que, no intuito de acolher as pessoas trans*, se coloca frente ao desafio de ampliar a oferta de procedimentos e serviços (para além dos voltados à readequação corporal) e, de oferecer maior abrangência às populações gênero dissidente (não bináries e outras), cujo cuidado continua a ser negligenciado. Analisou-se também a noção de integralidade, emergindo a coparticipação e simetria como atributos-desafios na dimensão relacional ou dos vínculos entre a medicina, seus profissionais e as pessoas usuárias. Finalmente, refletiu-se sobre a vulnerabilidade, enxergando nas condições precárias de instauração e funcionamento das políticas públicas um mecanismo tropológico, que junto aos (c)istemas médicos, reproduzem a vulnerabilidade por meio dos próprios mecanismos através dos quais tentam mitigar seus efeitos.

Abstract: This thesis focuses on the processes of meaning production around thetrans-specific health care (ATS), in order to question and give visibilityto the complexity of the ways in which trans-citizenship ought to beguaranteed. As part of the methodology, I established a dialogue andworked with the experiences of the users, medical staff and employeesof the municipal health secretary (SMS) linked to the service-assistanceof Florianópolis, as well as with the legal framework that guides theTransexualization Process in SUS and other policies/programs that serveas reference to the notions of ATS with which currently operates in thefield of health in Brazil.I conducted eleven interviews and used the concept of recursiveproblematization proposed by Fernández (2007) as a methodologicaloperator for the production of information, a choice that involves theelaboration of problem areas as part of the methodological approach.The analysis of the information produced in the field allowed an indepth and trans-situated reading of the demands that the users bring inthe encounter with these services and their implications in the face ofthe discussion of the meanings to which the ATS is linked.The ATS was discussed as a way of concretely addressing rights, albeitpartial, and their particularities in the context of APS and the Family andCommunity Health Strategy. Moreover, in meeting the challenge toaccommodate trans* people, the ATS faces the challenge of expandingthe offer of procedures and services (in addition to those aimed atcorporal rehabilitation) and of offering greater coverage to thepopulations of dissident (not binary and others) whose care remainsneglected. The notion of integrality was also analyzed, with coparticipation and symmetry emerging as attributes-challenges in therelational dimension or the links between medicine, its professionals andthe users. Finally, it was reflected on the vulnerability, seeing in theprecarious conditions of the establishment and operation of publicpolicies a tropological mechanism, which together with the medical(c)ystems reproduce the vulnerability through the very mechanismsthrough which they try to mitigate their effects.
