Impacto das sobredentaduras mandibulares implantorretidas na qualidade de vida e satisfação de pacientes edêntulos

Abstract

Objetivo: Comparar o grau de satisfação (GS) e o impacto na qualidade de vida (QV) de pacientes desdentados tratados com próteses totais convencionais (PTCs) e sobredenturas mandibulares (SMIs) retidas por 2 (Superfície de Ancoragem Reduzida - SAR) ou 4 (Superfície de Ancoragem Estendida - SAE) implantes. Métodos: Pacientes desdentados bimaxilares foram tratados com CCDs e posteriormente alocados randomicamente para receber uma sobredentadura mandibular retida pelo sistema barra-clipe em 2 implantes interforaminais (Grupo Controle) ou 4 (Grupo Teste), respectivamente. O questionário Oral Health Impact Profile Edent (OHIP-20E) e a Escala Analógica Visual (Visual Analogue Scale, VAS) foram aplicados para avaliar o impacto na QV e no GS, respectivamente. Comparações intra-indivíduos entre o tipo de prótese e inter-indivíduos entre a extensão da superfície de ancoragem foram realizadas com o teste t de Student para amostras independentes a um nível de significância de 5%. Resultados: As SIMs apresentaram valores de QV significativamente maiores nos domínios “desconforto psicológico” (p < 0,01), “incapacidade física” (p = 0,03) e “escore total” (p = 0,03) (n = 20 pacientes). As SIMs apresentaram valores de GS significativamente maiores nos domínios “fala” (p = 0,05), “conforto” (p = 0,03), “capacidade mastigatória” (p = 0,01), “função” (p <0,01) e “oral condição” (p = 0,03). O grupo SAR (controle) resultou em valores significativamente maiores no domínio “limitação funcional” (p <0,01) e na dimensão “capacidade de limpeza” (p = 0,01). Conclusões: As SIMs melhoraram significativamente a QV e a DS. O aumento da extensão da superfície de ancoragem não resultou em benefícios para a QV e o GS do paciente nesta análise transversal.

Aim: To compare the degree of satisfaction (DS) and the impact on the quality of life (QoL) of edentulous patients treated with conventional complete dentures (CCDs) and mandibular overdentures (MODs) retained by 2 (Short Antero-Posterior Spread, S-APS) or 4 (Large Antero-Posterior Spread, L-APS) implants. Methods: Bimaxillary edentulous patients were treated with CCDs and later randomly allocated to receive a bar-clip mandibular overdenture retained by 2 interforaminal (Control Group) or 4 (Test Group) implants, respectively. The Oral Health Impact Profile Edent (OHIP-20E) and the Visual Analogue Scale (VAS) questionnaire were applied to assess the impact on the QoL and the DS, respectively. Intra-individual comparisons between the type of prostheses and inter-individual comparisons between the AP spread extension were made with the Student t test for independent samples at a 5% significance level. Results: MODs showed significantly higher QoL values in the domains “psychological discomfort” (p< 0.01), “physical disability” (p = 0.03) and “overall score” (p = 0.03) (n = 20 patients). MODs showed significantly higher DS values in the domains “speech” (p = 0.05), “comfort” (p = 0.03), “chewing capacity” (p = 0.01), “function” (p<0.01) and “oral condition” (p = 0.03). The S-APS (control) group resulted in significantly higher values on the domain “functional limitation” (p<0.01) and on the dimension “cleaning ability” (p = 0.01). Conclusions: MODs significantly improved the QoL and DS. The increase of the AP spread did not result in benefits to the patient’s QoL and DS in this cross-sectional analysis.
