Significando a complexidade das interações profissionais na gestão da Educação Permanente em Saúde, segurança do paciente e qualidade em hospitais públicos

Abstract

A instituição hospitalar vem exigindo limites menos rígidos entre os diversos serviços na estrutura organizacional de forma a atender aos interesses globais da instituição e oportunizar um ambiente de contínua negociação. Sendo assim, o estudo buscou responder como acontece a interação profissional na estrutura organizacional hospitalar para a gestão da EPS a fim de garantir o cuidado de enfermagem seguro e qualificado e qual o significado dessa interação para os profissionais envolvidos. Objetivou compreender como acontece a interação profissional na estrutura organizacional hospitalar para a gestão da EPS a fim de garantir o cuidado de enfermagem seguro e qualificado a partir do significado atribuído pelos profissionais envolvidos. Parte-se da tese que os serviços que compõem a estrutura organizacional hospitalar, quando complementares e interdependentes, configuram uma única instância com potencialidade para interação profissional na gestão da EPS de forma a proporcionar um cuidado de enfermagem seguro e qualificado. Utilizou como referencial teórico a Teoria da Complexidade proposta por Edgar Morin e como referencial metodológico a vertente estruturalista da Teoria Fundamentada nos Dados. Participaram do estudo 27 profissionais, entre enfermeiros assistenciais, e profissionais do grupo gestor, dentre eles, coordenadores e gestores, os quais foram organizados em quatro grupos amostrais. As entrevistas ocorreram entre os meses de junho e dezembro de 2018 e iniciaram com uma questão norteadora. A pesquisa obedeceu à Resolução n.º 466/2012 do Conselho Nacional de Saúde do Ministério da Saúde. O processo de codificação aberta, axial e de integração foi realizado durante a coleta e análise dos dados, que ocorreu de forma concomitante e comparativa. Elaborou-se o paradigma com seus componentes (condição, ação-interação e consequência), categorias, subcategorias e categoria central, que representaram o fenômeno do estudo Significando a complexidade das interações profissionais na gestão da Educação Permanente em Saúde, segurança do paciente e qualidade em hospitais públicos e apresentaram a teoria substantiva. Esta foi apresentada no manuscrito Gestão da tríade Educação Permanente em Saúde, segurança do paciente e qualidade em hospitais públicos , o qual destacou as particularidades do sistema público de saúde, a influência do apoio do gestor e da prioridade de gestão, da disposição da estrutura organizacional, da cultura institucional, dos estímulos externos à instituição e, da iniciativa e liderança do enfermeiro na interação profissional para a gestão da tríade Educação Permanente em Saúde, segurança do paciente e qualidade, revelando a necessidade de uma mudança cultural por meio da interdisciplinaridade. Diante da necessidade de discutir com maior ênfase os significados das interações profissionais na gestão da Educação Permanente em Saúde, segurança do paciente e qualidade diante da relevância dessa perspectiva, o manuscrito Significados da interação profissional na gestão da Educação Permanente em Saúde, segurança do paciente e qualidade evidenciou a mobilização dos profissionais para a gestão da tríade Educação Permanente em Saúde, segurança do paciente e qualidade, sendo esta potencializada pela aproximação física, sensibilização, participação, comprometimento, responsabilização, motivação, liderança, autonomia, gestão compartilhada, comunicação efetiva, empatia, objetivos em comum e foco no paciente. Ademais, destacou a busca pela interdisciplinaridade, apresentando limitações, como a cultura institucional e a formação profissional; e estratégias, como espaços colegiados, envolvimento e cobrança do gestor, estímulos externos, além da participação de todos os atores na construção do planejamento e padronização e de instrumentos direcionados à atuação interdisciplinar. Diante dos resultados ampliou-se a concepção da tese com a seguinte estrutura: serviços que compõem a estrutura organizacional hospitalar, quando complementares e interdependentes, configuram uma instância com potencialidade para interação profissional na gestão da tríade EPS, segurança do paciente e qualidade, de forma a contribuir para o cuidado multiprofissional e interdisciplinar ao paciente. Ressalta-se a importância do ser humano e das relações interpessoais para a efetivação de processos de mudança nas instituições hospitalares, assim como a necessidade de novos modelos de gestão com ênfase em uma gestão mais participativa, que estimule a integração dos serviços e tenha por base a interação dos profissionais de forma horizontal e democrática, contribuindo desta forma para melhorar os processos de cuidado nas instituições hospitalares, com repercussões na rede de atenção à saúde.

Abstract: The hospital institution has required less severe restrictions among the various services in the organizational structure in order to meet the overall interests of the institution and provide an environment of continuous negotiation. Accordingly, the study sought to show the way in which the professional interaction occurs in the hospital organizational structure for the management of CHE in order to ensure a safe and qualified nursing care as well as the meaning of this interaction for the professionals involved. This study aimed to understand how professional interaction occurs in the hospital organizational structure for the management of CHE in order to ensure a safe and qualified nursing care based on the meaning attributed by the professionals involved. It starts from the thesis that the services that compose the hospital organizational structure, when complementary and interdependent, configure a particular instance with potential for professional interaction in the management of CHE, in such a way as to provide a safe and qualified nursing care. It used as theoretical framework the structuralist branch of the Complexity Theory proposed by Edgar Morin and as a methodological framework the Grounded Theory. The study was attended by 27 professionals, including health care nurses, and professionals from the managerial group, among which coordinators and managers, who were organized into four sampling groups. The interviews took place between the months of June and December 2018 and started with a guiding question. The research complied with Resolution nº. 466/2012 of the National Health Council of the Ministry of Health. The process of open and axial coding and integration was held during the collection and analysis of data, which took place in a concomitant and comparative way. The paradigm was designed with its components (condition, action-interaction and consequence), categories, subcategories and core category, which represented the phenomenon of the study Understanding the complexity of professional interactions in the management of Continuing Health Education, patient safety and quality in public hospitals and introduced the substantive theory. This was introduced in the manuscript named Management of the Continuing Health Education, patient safety and quality triad in public hospitals , which highlighted the particularities of the public health system, the influence of managerial support and managerial priority, the organizational structure arrangement, the institutional culture, the stimuli external to the institution and the initiative and leadership of the nurse in the professional interaction for the management of the Continuing Health Education, patient safety and quality triad, revealing the need for a cultural change through interdisciplinarity. Given the need to discuss with a greater emphasis the meanings of professional interactions in the management of the Continuing Health Education, patient safety and quality in the face of the relevance of this perspective, the manuscript named Meanings of professional interaction in the management of Continuing Health Education, patient safety and quality highlighted the mobilization of professionals for the management of the Continuing Health Education, patient safety and quality triad, which is strengthened by physical approach, awareness-raising, participation, commitment, responsibility, motivation, leadership, autonomy, shared management, effective communication, empathy, common goals and patient focus. Moreover, it highlighted the search for interdisciplinarity, showing limitations, such as institutional culture and professional training; and strategies, such as collegiate spaces, managerial involvement and collection, external stimuli, besides the participation of all actors in the design of planning and standardization and of instruments targeted at interdisciplinary action. Given the results obtained, the conception of the thesis was expanded with the following structure: services that compose the hospital organizational structure, when complementary and interdependent, configure an instance for professional interaction in the management of the CHE, patient safety and quality triad, in such a way as to contribute to offering a multidisciplinary and interdisciplinary care for the patient. It should be emphasized the importance of the human being and interpersonal relationships for the consolidation of change processes in hospital institutions, as well as the need for new management models with an emphasis on a more participative management, which should stimulate the integration of services and be based on the interaction of professionals in a horizontal and democratic way, thereby contributing towards the improvement of care processes in hospital institutions, with reverberations on the health care network.
