Adaptação transcultural e validação semântica da Vancouver Scar Scale - Baryza Version para a população brasileira

Abstract

Cicatrizes ocasionadas por queimaduras podem causar problemas estéticos, sintomáticos, psicológicos e funcionais, originando importante impacto na qualidade de vida de pessoas que sofreram queimaduras. Para o acompanhamento da evolução das mesmas e avaliação da eficácia do tratamento, uma variedade de escalas tem sido desenvolvida para cumprir um mesmo objetivo. Contrapondo-se à construção de uma nova escala, a adaptação transcultural de instrumentos torna-se necessária para atestar a qualidade das medidas em diferentes países, culturas e/ ou idiomas. A Vancouver Scar Scale Baryza Version, modificação de uma das escalas de avaliação de cicatrizes mais utilizadas no mundo, tem o objetivo de avaliar as principais características das cicatrizes, como pigmentação, vascularização, flexibilidade e altura, acrescida da resposta pigmentação mista , além de sugerir um instrumento de acrílico para auxiliar nas medições. Assim, observa-se a necessidade de aplicar os preceitos de adaptação transcultural sobre a Vancouver Scar Scale Baryza Version para ser utilizada na população brasileira de pessoas que sofreram queimaduras. O objetivo foi descrever a adaptação transcultural e validação semântica da Vancouver Scar Scale Baryza Version. Trata-se de estudo metodológico desenvolvido em sete etapas: 1) tradução da escala original em duas versões; 2) síntese das traduções; 3) avaliação por comitê de especialistas; 4) retrotradução (back-translation) em duas versões; 5) síntese das retrotraduções (back-translations); 6) comparação com a escala original; e 7) validação semântica. Na primeira etapa participaram dois tradutores brasileiros; na segunda, os pesquisadores e uma fisioterapeuta (mediadora) com experiência em queimaduras; na terceira, um médico, um fisioterapeuta e três enfermeiras; na quarta, dois nativos da língua inglesa; na quinta, os pesquisadores e uma enfermeira (mediadora); na sexta, o tutor da escala original; e na sétima, e última etapa, três enfermeiras e um médico, sem conhecimentos específicos sobre o tema. Ao longo do processo, foram apontadas dificuldades especialmente em diferenciar os itens pigmentação e vascularização e seus subitens. Foram necessários ajustes no título e nos subitens de flexibilidade , cujas traduções não se adequavam à prática clínica de avaliação de cicatrizes de queimaduras. Os resultados apontaram que as etapas de tradução e adaptação culminaram em um instrumento que foi conferido pelo tutor da escala original, sem necessidade de modificações, e submetido à validação semântica, onde a versão adaptada foi considerada boa/muito boa e muito importante para a condição em questão. Conclui-se que os objetivos dessa pesquisa foram alcançados, a escala foi traduzida e adaptada e a validação semântica obteve resultado satisfatório, resultando na versão brasileira preliminar da Vancouver Scar Scale Baryza Version.

Abstract : Scars caused by burns can cause aesthetic, symptomatic, psychological and functional problems, causing important impact on the quality of life of people who have suffered burns. In order to follow the evolution of the scars and to evaluate the effectiveness of the treatment, a variety of scales have been developed to fulfill the same objective. Contrary to the construction of a new scale, cross-cultural adaptation of instruments becomes necessary to attest to the quality of the measures in different countries, cultures and / or languages. The Vancouver Scar Scale - Baryza Version, a modification of one of the most used scoring scales in the world, aims to evaluate the main characteristics of scars, such as pigmentation, vascularity, flexibility and height, plus the mixed pigmentation response, besides suggesting an acrylic instrument to aid in measurements. Thus, it is observed the need to apply the precepts of cross-cultural adaptation on the Vancouver Scar Scale - Baryza Version to be used in the Brazilian population who have suffered burns. The aim was to describe the cross - cultural adaptation and semantic validation of the Vancouver Scar Scale - Baryza Version. It is a methodological study developed in seven stages: 1) translation of the original scale into two versions; 2) synthesis of translations; 3) evaluation by expert committee; 4) back-translation in two versions; 5) synthesis of back-translations; 6) comparison with the original scale; and 7) semantic validation. In the first stage participated two Brazilian translators; in the second, the researchers and a physiotherapist (mediator) with experience in burns; in the third, a doctor, a physiotherapist and three nurses; in the fourth, two natives of the English language; in the fifth, the researchers and a nurse (mediator); in the sixth, the tutor of the original scale; and in the seventh, and last stage, three nurses and a doctor, without specific knowledge on the subject. Throughout the process, difficulties were pointed out especially in differentiating the items \"pigmentation\" and \"vascularization\" and its subitems. Adjustments were required in the title and in the sub-items of \"flexibility\", whose translations were not adequate to the clinical practice of evaluation of burn scars. The results showed that the translation and adaptation stages culminated in an instrument that was conferred by the tutor of the original scale, without any modifications, and submitted to semantic validation, where the adapted version was considered good / very good and very important for the condition in question. It is concluded that the objectives of this research were achieved, the scale was translated and adapted and the semantic validation obtained a satisfactory result, resulting in the preliminary Brazilian version of the Vancouver Scar Scale - Baryza Version.
