Uma teoria substantiva acerca da acreditação hospitalar em um hospital público

Abstract

Este estudo apresenta uma teoria substantiva, aplicada nos processos deimplantação, manutenção e evolução da acreditação hospitalar. Apesquisa foi realizada entre os anos de 2015 a 2017, por meio do case doHospital Regional Público da Transamazônica, localizado em Altamira(PA), região Norte do Brasil. Os argumentos, de ordem pessoal, práticae teórica motivaram este estudo, cujo objetivo é compreender aimplantação, manutenção e evolução da acreditação hospitalar,desenvolvendo uma teoria substantiva. Assim, na busca pelacompreensão de um contexto específico, este estudo foi conduzido peloparadigma interpretativo, em uma abordagem construtivista. Foi adotadacomo abordagem de pesquisa, seguindo os pressupostos de Charmaz(2009), a grounded theory, por ter se mostrado adequada, visto que seusprocedimentos visam, por meio da interpretação dos envolvidos napesquisa bem como do pesquisador, construir uma resposta conjuntasobre o fenômeno investigado. Os dados foram coletados por meio deentrevistas, observação participante, conversas de cumbuca e análisedocumental, em cinco rodadas distintas, intercaladas com análises evalidação dos dados. Ao todo, 56 atores sociais, envolvidos no contextode estudo, direta ou indiretamente, participaram por meio de reflexõessobre o fazer organizacional empreendido no hospital em análise. Asreflexões conduziram a um modelo sistêmico denominado Comprometimento com o cuidado , que é permeado por três categoriascentrais: A forma como nós trabalhamos , Acreditação comoinstrumento de ação e Amor à causa ; uma categoria de suporte: Desenvolvimento dos profissionais de saúde ; e, duas categorias deresultado: Excelência nos resultados e Cuidado centrado no paciente .Essas categorias impactam as subcategorias e estas, entre si, delineandoa estrutura com 39 proposições vinculadas. Juntas, categorias eproposições formam uma teoria substantiva, na qual se estabelece a tesede que a acreditação hospitalar é implantada, que se mantém e evolui emresposta aos determinantes externos, os quais induzem à busca decontingências para solucionar pressões humanas, sociais e econômicas,oriundas, especialmente, das instituições presentes no ambiente e frutoda localização, que, associadas às fragilidades internas do processo,determinam a forma como os atores sociais atuam. A atuação dessesatores se alinha à melhoria contínua, como um instrumento do agircorretamente, e se fortalece do elo emotivo gerado nos profissionais pelanecessidade do outro, pela sua própria necessidade e a dos seus. Outrospontos que se destacaram dizem respeito à implantação do projeto, aosprofissionais que vieram de outros Estados recém-formados, aoreconhecimento das fragilidades, à posição do hospital como apoiadorda rede de saúde e ao amor que permeia a complexidade instalada. Esseamor está vinculado à forma como o hospital se constitui; ao entremeiode profissionais, que, com várias visões de mundo, criaram uma paraexplicar aquele contexto; à aceitação de que culturas diferentes podemcoexistir e se apoiarem; à vontade de fazer o bem ao outro e ver que issofavorece o sentido de que o profissional pertence àquele lugar e quecontribui para a mudança daquele espaço; ao estar junto ao paciente deforma mais integral, trazendo-o à discussão de seu próprio cuidado,tornando-o agente de seu restabelecimento à saúde e à sociedade; àsrelações estabelecidas e à significância que o hospital tem para aspessoas e que o consideram como a causa de suas vidas. Esses achadosdialogaram com o pensamento complexo e o estudo das instituições. Omodelo proposto se mostrou uma alternativa integradora e sistêmica àcompreensão de como ocorre a acreditação hospitalar na organizaçãoestudada. O resultado encontrado pode motivar novas pesquisas,visando desenvolver teorias no contexto brasileiro.

Abstract : This study presents a substantive theory applied in the implantation,maintenance and evolution processes of hospital accreditation. Thestudy was carried out between 2015 and 2017, through the case of theTransamazônica Regional Public Hospital, located in Altamira (PA),northern region of Brazil. The arguments, personal, practical andtheoretical, motivated this study, whose objective is to understand theimplementation, maintenance and evolution of hospital accreditation,developing a substantive theory. Thus, in the search for theunderstanding of a specific context, this study was led by theinterpretative paradigm, in a constructivist approach. It was adopted as aresearch approach, following the assumptions of Charmaz (2009),grounded theory, for having proved adequate, since its procedures aim,through the interpretation of those involved in the research as well as theresearcher, to build a joint response on the phenomenon investigated.Data were collected through interviews, participant observation,cumbuca conversations and documentary analysis, in five differentrounds, interspersed with data analysis and validation. In all, 56 socialactors, involved in the study context, directly or indirectly, participatedthrough reflections on the organizational work undertaken in thehospital under analysis. The reflections led to a systemic model called"Commitment to Care," which is permeated by three central categories:'The way we work', 'Accreditation as an instrument of action' and 'Loveto the cause'; a category of support: 'Development of healthprofessionals'; and two outcome categories: 'Excellence in Outcomes'and 'Patient-Centered Care'. These categories impact the subcategoriesand these, among them, outlining the structure with 39 relatedpropositions. Together, categories and propositions form a substantivetheory, in which the thesis is established that hospital accreditation isimplanted, maintained and evolved in response to external determinants,which induce the search for contingencies to solve human, social andeconomic pressures, especially from the institutions present in theenvironment and the result of the location, which, together with theinternal fragilities of the process, determine the way in which socialactors act. The performance of these actors aligns with continuousimprovement, as an instrument of acting correctly, and is strengthenedby the emotional bond generated in professionals by the need of theother, by their own need and their own. Other points that stood outconcern the implementation of the project, the professionals who camefrom other newly formed states, the recognition of fragilities, thehospital's position as a supporter of the health network and the love thatpermeates the installed complexity. This love is linked to the way thehospital is constituted; to the entremeio of professionals, who, withvarious worldviews, created one to explain that context; the acceptancethat different cultures can coexist and support each other; the will to dogood to the other and see that it favors the sense that the professionalbelongs to that place and that contributes to the change of that space; bybeing with the patient in a more integral way, bringing him to thediscussion of his own care, making him the agent of his recovery tohealth and society; the relationships established and the significance thatthe hospital has for people and who consider it as the cause of theirlives. These findings were in dialogue with complex thinking and thestudy of institutions. The proposed model was an integrative andsystemic alternative to the understanding of how hospital accreditationoccurs in the organization studied. The result can motivate new research,aiming to develop theories in the Brazilian context.
