Introdução: A participação de mulheres no meio esportivo cresceu consideravelmente, principalmente em atividades que levam ao aumento da pressão intra-abdominal (PIA). Acredita-se que esse aumento, a fadiga neuromuscular gerada pela intensidade dos treinos e a sobrecarga no assoalho pélvico (AP) podem relacionar-se às disfunções do assoalho pélvico (DAP); porém, na literatura, existem poucos estudos sobre o comportamento do AP após a realização do treinamento de força. Objetivo: Analisar o efeito imediato da prática de levantamento de peso na função dos músculos do AP de mulheres adultas que realizam treinamento de força. Método: Estudo clínico randomizado, aberto com desenho cruzado. Incluiu mulheres adultas que realizavam treinamento de força no mínimo duas vezes por semana, a pelo menos três meses, nulíparas com idade entre 18 e 35 anos. As mulheres receberam um questionário (link via Google Forms®) para definir a elegibilidade ao estudo e a caracterização da amostra. Para determinar a presença e o grau do incômodo gerado pelas DAP foi utilizado o Pelvic Floor Distress Inventory (PFDI-20). Após a finalização desta primeira etapa, as participantes passaram por uma avaliação funcional do AP, através da Escala de Oxford Modificada e da eletromiografia de superfície (NewMiotool Miotec®). As mulheres foram randomizadas de forma aleatória em grupo intervenção (GI) e grupo controle (GC). A intervenção do estudo foi composta por exercícios de mobilidade, corrida estacionária, agachamento livre com barra e levantamento terra. Foram realizadas 7 séries dos exercícios de levantamento de peso, com 20%, 44%, 75%, 85%, 93%, 95% e 100% da carga máxima utilizada por cada voluntária. O GI passou por avaliação pélvica inicial, seguido pela intervenção, reavaliação imediata do AP (T1) e reavaliações aos 60 minutos (T2) e 30 minutos (T3), totalizando 90 minutos de acompanhamento após intervenção. O GC iniciou com avaliação pélvica, permaneceu 60 minutos em repouso, reavaliadas imediatamente (T1) e submetidas à intervenção, após os exercícios, o GC foi reavaliado imediatamente e 30 minutos depois. Os dados foram tratados por meio de estatística descritiva e inferencial, com nível de significância de 5%. Resultados: Nove mulheres fizeram parte deste estudo, com mediana de idade de 25 anos (IA: 6,0). Seis participantes apresentaram sinais e sintomas de DAP, classificados por meio do PFDI- 20 e foram randomizadas em GI (n=4) e GC (n=5). O GC possuí frequência de treino semanal maior quando comparado ao GI (p=0,016), bem como IMC menor Md: 24,42 (Ai:10,77). A análise temporal não revelou diferenças estatisticamente significativas entre T1, T2 e T3 para força, ativação muscular e fadiga dos músculos do assoalho pélvico, tanto no GI quanto (p=0,440; p=0,552) no GC (p=0,516; p=0,472). Quando a frequência mediana (FM) foi analisada individualmente, observou-se que três participantes do GI apresentaram sinais de fadiga muscular em T1, T2 e T4; as quais também apresentavam IMC elevado e menor frequência semanal de treinamento. Conclusão: Os resultados indicam que uma única sessão de levantamento de peso, realizada em intensidade semelhante àquela praticada rotineiramente pelas voluntárias, não promoveu alterações agudas significativas na função objetiva dos MAPs. Bem como, os achados indicam respostas neuromusculares individuais, incluindo fadiga muscular, evidenciando a complexidade da relação entre o treinamento de força e a função do assoalho pélvico. Abstract: Introduction: Women's participation in sports has grown considerably, especially in activities that lead to increased intra-abdominal pressure (IAP). It is believed that this increase, the neuromuscular fatigue generated by the intensity of training, and the overload on the pelvic floor (PF) may be related to pelvic floor dysfunction (PFD); however, there are few studies in the literature on the behavior of the PF after strength training.Objective: To analyze the immediate effect of weightlifting on the function of the AP muscles in adult women who perform strength training. Method: This was a randomized, open-label, crossover clinical trial. It included adult women who had been performing strength training at least twice a week for at least three months, who were nulliparous and aged between 18 and 35 years. The women received a questionnaire (link via Google Forms®) to determine eligibility for the study and to characterize the sample. To determine the presence and degree of discomfort caused by PAD, the Pelvic Floor Distress Inventory (PFDI-20) was used. After the completion of this first stage, the participants underwent a functional assessment of the pelvic floor using the Modified Oxford Scale and surface electromyography (NewMiotool Miotec®). The women were randomly assigned to an intervention group (IG) and a control group (CG).The study intervention consisted of mobility exercises, stationary running, free squats with a barbell, and deadlifts. The study intervention consisted of mobility exercises, stationary running, free squats with a barbell, and deadlifts. Seven sets of weightlifting exercises were performed, with 20%, 44%, 75%, 85%, 93%, 95%, and 100% of the maximum load used by each volunteer. The GI underwent initial pelvic assessment, followed by the intervention, immediate reassessment of the AP (T1), and reassessments at 60 minutes (T2) and 30 minutes (T3), totaling 90 minutes of follow- up after the intervention.The control group (CG) began with a pelvic assessment, remained at rest for 60 minutes, was immediately reassessed (T1), and underwent the intervention. After the exercises, the CG was reassessed immediately and 30 minutes later. Data were analyzed using descriptive and inferential statistics, with a significance level of 5%. Results: Nine women participated in this study, with a median age of 25 years (AI: 6.0). Six participants presented signs and symptoms of PAD, classified using the PFDI-20, and were randomized into GI (n=4) and GC (n=5).The GC group has a higher weekly training frequency compared to the GI group (p=0.016), as well as a lower BMI Md: 24.42 (Ai:10.77). Temporal analysis did not reveal statistically significant differences between T1, T2, and T3 for strength, muscle activation, and fatigue of the pelvic floor muscles, both in the GI and in the GC (p=0.516; p=0.472) groups. When the median frequency (FM) was analyzed individually, it was observed that three participants in the GI group showed signs of muscle fatigue at T1, T2, and T4; these participants also had a high BMI and a lower weekly training frequency. Conclusion: The results indicate that a single weightlifting session, performed at an intensity similar to that routinely practiced by the volunteers, did not promote significant acute changes in the objective function of the pelvic floor muscles. Likewise, the findings indicate individual neuromuscular responses, including muscle fatigue, highlighting the complexity of the relationship between strength training and pelvic floor function. |
