Gays e o novo sexo seguro: a TARV, a PrEP e a limitrofia no dispositivo da aids

Abstract

Desde uma mirada genealógica e neomaterialista, investigo, nesta tese, a relação sujeito-ARVs (TARV e PrEP) em um campo de agenciamento mútuo e ?devir com?, tendo em vista deslocamentos recentes no dispositivo da aids e suas repercussões ao uso do prazer entre gays cis usuários dos fármacos em Florianópolis (SC). Conduzi, para tanto, durante o ano de 2023, grupos focais no Grupo de Apoio e Prevenção à Aids (GAPA) e realizei entrevistas com 16 usuários gays cis de PrEP ou TARV em tratamento no município, entre 2022 e 2023. Em relação à PrEP, faço notar que é o ?medo do hiv? que deslinda, para gays cis, o portal terapêutico em direção à profilaxia, prevalecendo sobre uma hesitação inicial manifestada pelo ?preconceito? devido à capacidade do fármaco de caracterizar um comportamento promíscuo e, supostamente, destravar outros riscos, assim como pela preocupação com seus efeitos colaterais, memória tóxica dos ARVs do último século, aqui retomada. Aponto ainda que o monitoramento clínico e laboratorial a que são submetidos quando usuários é cooptado, no entanto, a uma gestão da saúde sexual mais ampla, que reitera a agência securitária do fármaco e é descrita como facilitadora de seu acesso à saúde e propulsora de uma lógica de confiança e cuidado. Em se tratando dos usuários de TARV, a indetectabilidade se apresenta não somente como o biomarcador supremo da terapêutica, mas também aglutina as concepções de saúde correntes, embora não prescinda de outras rotinas de disciplina corporal e investimento sobre si, alocadas a um ?estilo de vida? e descritas também como um cuidado. Já sua ligação ao sexo, na forma da intransmissibilidade, expõe experiências cesuradas: práticas de outrora, de risco ?mínimo? e injunção à confissão, mantém-se preservadas e cortes agenciais cindem as relações entre um fármaco que seria voltado ao tratamento e à manutenção da vida (TARV) e outro à prevenção e à produção de prazer (PrEP). É através da pergunta ?Toma PrEP??, recorrente entre gays cis e que elege a profilaxia como correlato de ?sexo seguro?, que o hiv, a sorologia e a relação com o fármaco são pautados nessas relações e utilizados para definir seus desfechos preventivos. Quando direcionada à PVHIV, observo que a questão a confronta com sua produção sorofarmacológica diferencial neste dispositivo e libera uma agonística, uma relação diante da verdade que tenta ser resolvida, em geral, com um certo pragmatismo, que justifica o consentimento à interrogação em situações de sexo casual e, com isso, induz o indetectável ao apagamento nos arranjos interfarmacológicos recentes. A pergunta, contudo, serviria também à ostentação da soronegatividade por parte de seus enunciadores, delimitando uma linha somática que busca separar os corpos, agora igualmente medicalizados, ?com vírus? daqueles ?sem vírus?. Enquanto topologia, atento que o barebacking, por meio de seus significados ?excitáveis?, relacionados à fantasia de um sexo não-mediado, organiza espaços de conectividade entre o orgânico e o inorgânico, a natureza e a cultura, o humano e não-humano que ameaçam ascender nos emaranhados compostos por tecnologias biomédicas, hiv e corpos de gays cis em interação cruzada em Florianópolis.

Abstract: In this thesis I investigate, from a genealogical and neo-materialist perspective, the subject-ARVs (HAART and PrEP) relationship in a field of mutual agency and ?becoming with?, in view of recent shifts in the aids apparatus and its backlashes on the use of pleasure among cis gay users of such drugs in Florianópolis (SC). To meet that end, in 2023 I conducted focus groups at the Aids Support and Prevention Group (GAPA) and carried out interviews with 16 gay cis users of PrEP or HAART undergoing treatment in the municipality between 2022 and 2023. With regard to PrEP, I point out that it is the ?fear of hiv? that opens the therapeutic portal towards prophylaxis for cis gay men, prevailing over an initial hesitation manifested by ?prejudice? due to the drug's ability to characterize promiscuous behavior and supposedly unlock other risks, as well as the concern about its side effects, a toxic memory of the ARVs of the last century, which is revisited here. I also point out that the clinical and laboratory monitoring to which they are subjected as users is co-opted, however, into a broader sexual health management, which reiterates the security agency of the drug and is described as facilitating their access to health and propelling a logic of trust and care. When it comes to HAART users, undetectability is not only the supreme biomarker of therapy, but it also brings together current conceptions of health, although it does not dispense with other routines of bodily discipline and investment in oneself, allocated to a ?lifestyle? and also described as care. On the other hand, its link to sex, in the form of non-transmissibility, exposes fractured experiences: past practices, of ?minimal? risk and injunction to confess, are preserved and agential cuts split the relationship between a drug that would be aimed at the treatment and maintenance of life (HAART) and another aimed at prevention and the production of pleasure (PrEP). It is through the question ?Do you take PrEP??, which is recursive among cis gay men and who choose prophylaxis as a correlate of ?safe sex?, that hiv, serology and the relationship with the drug are guided by in these relationships and used to define their preventive outcomes. When directed at PLVIH, I note the question confronts them with their differential seropharmacological production in this device and releases an agonistic, a relationship with the truth that tries to be resolved, in general, with a certain pragmatism, which justifies consent when questioned in situations of casual sex and, thus, induces the undetectable to be erased in recent inter-pharmacological arrangements. The question, however, would also serve to boast the seronegativity of its enunciators, delimitating a somatic line aimed at separating bodies, now equally medicalized, ?with viruses? from those ?without viruses?. As a topology, I point out that barebacking, through its ?excitable? meanings related to the fantasy of unmediated sex, organizes spaces of connectivity between the organic and the inorganic, nature and culture, the human and the non-human that threaten to rise in the entanglements made up of biomedical technologies, hiv and the bodies of cis gay men in cross-interaction in Florianópolis.
