Introdução: Segundo o Relatório Global de Segurança do Paciente (2024), aproximadamente 40% dos pacientes atendidos na atenção primária estão potencialmente sujeitos a danos, em decorrência de eventos adversos. Com isso, esse projeto investiga a cultura de segurança do paciente na Atenção Primária à Saúde nos municípios de Manaus e Coari, ambos localizados no estado do Amazonas. Objetivo geral: Analisar a cultura de segurança do paciente na atenção primária à saúde em municípios do Amazonas. Os objetivos específicos foram: mensurar o clima de segurança do paciente percebido pelos profissionais da equipe de saúde por meio do questionário Medical Office Survey on Patient Safety Culture; comparar os escores das dimensões do questionário com as variáveis sociodemográficas; elencar temas prioritários para a segurança do paciente com base nos resultados dos domínios do questionário sobre cultura de segurança. Método: O delineamento da pesquisa foi quantitativo, transversal, analítico do tipo survey, com base no instrumento Medical Office Survey on Patient Safety Culture. Foi desenvolvida em duas unidades básicas de saúde, sendo uma de um município ribeirinho (A) e a outra da capital do Amazonas (B). A coleta de dados ocorreu via formulário online no período de setembro a novembro de 2025, cujos participantes compreenderam a equipe pertencente aos serviços. A análise descritiva dos dados seguiu o cálculo de frequência de resposta dos itens e média das dimensões do instrumento, sendo estas classificadas como fortes, intermediárias ou fracas. O teste Alfa de Cronbach avaliou a consistência interna e o teste de Kruskal-Wallis a comparação entre as variáveis. A pesquisa seguiu os preceitos éticos preconizados. Resultados: A unidade A demonstrou que a maioria dos 25 participantes foram mulheres (72%), com idade entre 36 a 45 anos (44%), com menos de 3 anos no serviço (32%), cumprindo carga horária de 33 a 40 horas semanais (44%). Entre as 12 dimensões do questionário, três foram classificadas como fortes, oito intermediárias e uma fraca. Em relação à unidade B, a maioria dos 42 participantes foram mulheres (73,8%), com idade entre 46 e 55 anos (47,6%), ensino superior completo (33,3%), técnicos de enfermagem (52,4%), com tempo de atuação entre três e seis anos (35,7%), cumprindo carga horária de 33 a 40 horas semanais (59,5%). Entre as 12 dimensões do questionário, três foram classificadas como fortes, oito intermediárias e uma fraca. Na avaliação global da qualidade de cuidados de saúde na unidade A e B, a classificação ?Bom? apresentou a maior frequência (46%) e (42,9%), respectivamente. O tempo de serviço apresentou influência estatisticamente significativa na percepção da cultura de segurança na unidade A e na unidade B, assim como as variáveis escolaridade e categoria profissional. Conclusão: Em ambas as unidades básicas de saúde a cultura de segurança do paciente foi classificada como intermediária. As dimensões fortes em ambas as unidades foram Trabalho em Equipe, Aprendizagem Organizacional e Acompanhamento e Cuidado do Paciente. Na unidade B, acrescenta-se a dimensão Percepção Geral de Segurança do Paciente e Qualidade. Nas unidades A e B, a dimensão classificada como fraca foi Pressão e Ritmo de Trabalho. Constatou-se potencial para consolidar uma cultura de segurança forte, mediante ações estruturantes que reduzam a sobrecarga e reforcem rotinas de comunicação efetiva, aprendizagem e gerenciamento de riscos, por meio de intervenções organizacionais que assegurem melhores condições de trabalho e valorização das equipes. |

Abstract: Introduction: According to the Global Patient Safety Report (2024), approximately 40% of patients receiving primary care are at risk of harm due to adverse events. Consequently, this project investigates the patient safety culture in primary health care in the municipalities of Manaus and Coari, both located in the state of Amazonas. General objective: To analyze the patient safety culture in primary health care in municipalities of Amazonas. The specific objectives were: to measure the patient safety climate as perceived by healthcare team professionals using the Medical Office Survey on Patient Safety Culture; to compare the scores of the questionnaire?s dimensions with sociodemographic variables; to list priority themes for patient safety based on the results of the questionnaire?s domains on safety culture. Method: The study design was quantitative, cross-sectional, and analytical (survey-based), using the Medical Office Survey on Patient Safety Culture. It was conducted in two primary health care units, one in a riverside municipality (A) and the other in the capital of Amazonas (B). Data collection took place via an online form from September to November 2025, with participants comprising the staff of the respective units. Descriptive data analysis involved calculating the frequency of responses to the items and the mean scores for the instrument?s dimensions, which were classified as strong, intermediate, or weak. Cronbach?s alpha was used to assess internal consistency, and the Kruskal-Wallis test was used to compare the variables. The study adhered to established ethical guidelines. Results: Unit A showed that the majority of the 25 participants were women (72%), aged 36 to 45 years (44%), with less than 3 years of service (32%), working 33 to 40 hours per week (44%). Among the 12 dimensions of the questionnaire, three were classified as strong, eight as moderate, and one as weak. Regarding Unit B, the majority of the 42 participants were women (73.8%), aged between 46 and 55 years (47.6%), with a college degree (33.3%), nursing technicians (52.4%), with three to six years of experience (35.7%), working 33 to 40 hours per week (59.5%). Among the 12 dimensions of the questionnaire, three were classified as strong, eight as moderate, and one as weak. In the overall assessment of the quality of healthcare in Units A and B, the ?Good? rating was the most frequent (46%) and (42.9%), respectively. Length of service had a statistically significant influence on the perception of safety culture in Unit A and Unit B, as did the variables of educational level and professional category. Conclusion: In both primary health care units, the patient safety culture was classified as moderate. The strong dimensions in both units were Teamwork, Organizational Learning, and Patient Monitoring and Care. In Unit B, the dimensions General Perception of Patient Safety and Quality were also strong. In Units A and B, the dimension classified as weak was Work Pressure and Pace. Potential was identified to consolidate a strong safety culture through structural measures that reduce workload and reinforce routines of effective communication, learning, and risk management, via organizational interventions that ensure better working conditions and team recognition. |
