Introdução: Os indivíduos que possuem algum tipo de disfunção temporomandibular (DTM) geralmente apresentam limitações na função mastigatória, levando a dificuldade para ingerir alimentos mais consistentes e cansaço na musculatura orofacial após a mastigação. Devido a isto, os hábitos e preferências alimentares podem ser influenciados, assim como alterações na antropometria facial, na oclusão e na avaliação de motricidade orofacial podem ser encontradas. Objetivo: Descrever as preferências e hábitos alimentares e compará-los com a antropometria facial e as características clínicas da mastigação de indivíduos que apresentam diagnóstico de DTM. Método: Estudo do tipo caso-controle no qual a amostra foi constituída por conveniência por 20 indivíduos com DTM de origem muscular ou mista (GDTM) e 20 indivíduos que não apresentavam DTM - grupo controle (GC). Os critérios de inclusão foram adultos com faixa etária entre 18 e 50 anos apresentando dentição completa, com diagnóstico de DTM, sem queixas fonoaudiológicas relacionadas à alimentação (mastigação e deglutição). Foram excluídos os indivíduos com alterações neurológicas, motoras, em uso de próteses dentária total ou parcial, com prótese total sobre implante, ausência de dentes em pares de oclusão e/ou de mais de três elementos dentários, oclusão em classe II ou III ou com histórico de paralisia facial, indivíduos que estivessem em uso de medicamentos relaxantes musculares ou que causassem xerostomia, com diagnóstico de DTM articular exclusiva e ou que haviam realizado tratamento para DTM nos últimos 6 meses. Para participar do estudo, os indivíduos foram solicitados a responder um questionário com perguntas sobre dados antropométricos, características e sintomas orofaciais, hábitos e preferências alimentares e classificação de consistências de alimentos apresentados. Os indivíduos que completaram satisfatoriamente os critérios de inclusão e responderam ao questionário completo foram convidados a participar da avaliação clínica odontológica e fonoaudiológica utilizando os protocolos DC/TMD e MBGR, respectivamente. Resultados: Os indivíduos com DTM buscam com maior frequência por alimentos mais fáceis de mastigar e classificam alguns alimentos como mais duros do que o GC, mas os hábitos alimentares não foram significativamente diferentes entre os grupos (p=0,301). Em relação a antropometria de face, não houve diferença significativa entre os lados da face. Encontrou-se medida de abertura máxima de boca e de lateralidade mandibular menores para o GDTM. A medida da DVO foi maior no GDTM. A mastigação bilateral foi predominante nos dois grupos e apenas no GDTM houve a classificação de mastigação unilateral crônica. Conclusão: Os hábitos alimentares não apresentaram expressiva variação entre os grupos em relação à escolha dos alimentos, mas sim em relação à frequência de consumo. Os indivíduos com DTM tendem a preferir consistências alimentares mais macias, mas o GC também apresentou preferência a alimentar-se, principalmente, com alimentos classificados como moles. A preferência por alimentos moles pode contribuir para alterações na musculatura orofacial, conforme identificado na avaliação de motricidade orofacial neste estudo. Em relação à antropometria de face, não houve diferença significativa entre os lados da face, indicando assim tendência a simetria facial nos indivíduos que participaram deste estudo. No que se refere a avaliação intra oral encontrou-se abertura máxima de boca e medida de lateralidade mandibular menores para o GDTM. A mastigação bilateral alternada foi predominante nos dois grupos e apenas no GDTM houve a classificação da mastigação unilateral crônica. |

Abstract: Introduction: Individuals who have some type of temporomandibular disorder (TMD) generally have limitations in their chewing function, leading to difficulty eating more consistent foods and fatigue in the orofacial muscles after chewing. Due to this, eating habits and preferences can be influenced, as well as changes in facial anthropometry, occlusion and assessment of orofacial motricity can be found. Objective: To describe eating preferences and habits and compare them with facial anthropometry and clinical chewing characteristics of individuals diagnosed with TMD. Method: Case-control study in which the convenience sample consisted of 20 individuals with TMD of muscular or mixed origin (GDTM) and 20 individuals who did not have TMD - control group (CG). The inclusion criteria were adults aged between 18 and 50 years old, with complete dentition, diagnosed with TMD, without speech-language complaints related to eating (chewing and swallowing). Individuals with neurological and motor disorders, using total or partial dental prostheses, with total prosthesis on implant, absence of teeth in occlusion pairs and/or of more than three dental elements, class II or III occlusion or with history of facial paralysis, individuals who were using muscle relaxing medications or medications that caused xerostomia, diagnosed with exclusive joint TMD and/or who had undergone treatment for TMD in the last 6 months. To participate in the study, individuals were asked to answer a questionnaire with questions about anthropometric data, orofacial characteristics and symptoms, eating habits and preferences and classification of food consistencies presented. Individuals who satisfactorily completed the inclusion criteria and responded to the full questionnaire were invited to participate in the dental and speech therapy clinical evaluation using the DC/TMD and MBGR protocols, respectively. Results: Individuals with TMD more frequently seek foods that are easier to chew and classify some foods as harder than the CG, but eating habits were not significantly different between the groups (p=0.301). Regarding facial anthropometry, there was no significant difference between the sides of the face. Lower maximum mouth opening and mandibular laterality measurements were found for the GDTM. The DVO measurement was higher in the GDTM. Bilateral chewing was predominant in both groups and only in GDTM was chronic unilateral chewing classified. Conclusion: Eating habits did not show significant variation between groups in relation to food choice, but rather in relation to frequency of consumption. Individuals with TMD tend to prefer softer food consistencies, but the CG also showed a preference for eating mainly foods classified as soft. The preference for soft foods may contribute to changes in orofacial muscles, as identified in the assessment of orofacial motricity in this study. Regarding facial anthropometry, there was no significant difference between the sides of the face, thus indicating a tendency towards facial symmetry in the individuals who participated in this study. Regarding intraoral evaluation, maximum mouth opening and mandibular laterality measurements were found to be smaller for the GDTM. Alternating bilateral chewing was predominant in both groups and only in GDTM was chronic unilateral chewing classified. |
