Saúde bucal de idosos com 80 anos ou mais no município de Antônio Carlos, Santa Catarina

Abstract

Objetivou-se comparar a condição, a autopercepção de saúde bucal e o padrão de utilização de serviços odontológicos de idosos, com 80 anos ou mais, em Antônio Carlos (Santa Catarina), nos anos 2011 e 2015. Trata-se de estudo com base domiciliar, longitudinal, descritivo, de abordagem quantitativa. Participaram 59 idosos, em 2011, que foram reexaminados em 2015. Foi aplicado uma versão adaptada do questionário utilizado no levantamento SB BRASIL 2010, complementado com índice OHIP-14 (Oral Impact Health Profile). Em ambos os anos investigados, o estudo constatou alta prevalência de idosos com 80 anos ou mais edêntulos e com limitada reabilitação protética. Observou-se padrões de higiene bucal deficientes, e consequentemente um aumento em 27% na presença de placa bacteriana visível a olho nu entre os exames. Outro aspecto clínico foi a diminuição significativa no número de dentes presentes no arco superior. Referente à utilização dos serviços odontológicos, o motivo da última consulta manteve-se semelhante em ambos os exames, sendo a confecção de próteses dentárias a principal razão apontada. Todavia, mesmo tratando-se do principal motivo, houve um aumento de 15,6% na necessidade de algum tipo de prótese nova inferior. Além disso, houve uma redução de 15,3% nas consultas realizadas por motivo de rotina. A sensação de boca seca aumentou 26,4%, e o desconforto para comer 35,5%. Conhecer as características da saúde bucal dos idosos nessa fase avançada da vida constitui-se uma oportunidade para se organizar a assistência a esse grupo etário de acordo com as suas reais necessidades, tornando possível assim, o planejamento e implementação de políticas públicas efetivas, que atendam as demandas da população idosa mais frágil, no âmbito da saúde bucal.

This study aimed to compare the condition, the self-perception of oral health and the pattern of use of dental services for the elderly with 80 years or more in Antônio Carlos (Santa Catarina) in the years 2011 and 2015. This is a home-based study, longitudinal, descriptive with a quantitative approach. Participated 59 elderly in 2011, which were reexamined in 2015. It was applied an adapted version of the questionnaire used in the SB BRAZIL 2010 survey, complemented by OHIP-14 index (Oral Health Impact Profile). In both investigated years, the study found a high prevalence of edentulous elderly aged 80 or more and with limited prosthetic rehabilitation. It was observed poor oral hygiene standards and an increase of 27% in the presence of visible dental plaque between examinations. Another clinical aspect was the significant decrease in the number of teeth present in the upper arch. Regarding the use of dental services, the reason for the last visit remained similar in both exams, being the production of dental prostheses the main reason. However, even though this is the main reason, there was a 15.6% increase in the need for inferior new prosthesis. In addition, there was a reduction of 15.3% in consultations for routine reasons. The sensation of dry mouth increased 26.4%, and the discomfort to eat 35.5%. Knowing the characteristics of oral health of the elderly in this advanced stage of life constitutes an opportunity to organize dental care for this age group according to their actual needs. And also making possible the planning and implementation of effective public policies that meet the demands of the more fragile elderly population, in the context of oral health.
