Principais falhas e queixas de pacientes em próteses fixas sobre implantes - revisão de literatura

Abstract

Um dos desafios da odontologia é reestabelecer estética, função e saúde ao paciente edêntulo. Dentre as opções de tratamento a prótese total fixa sobre implante surge como uma alternativa altamente previsível. Mesmo assim, continua apresentando pequenas porcentagens de falhas, que mesmo minoritárias, trazem aos pacientes sofrimento,transtornos e insatisfação. O objetivo desse trabalho foi de analisar na literatura quais são as principais falhas mecânicas e biológicas encontradas, bem como quais são as queixas mais frequentes nos pacientes portadores de próteses totais fixas sobre implantes. A pesquisa foi feita nas bases de dados como Scielo, Pubmed e portal de periódicos da Capes. Foram selecionados artigos em inglês e português publicados entre 2006 e 2013 para a redação desta revisão de literatura. Após a revisão de literatura pode-se observar que se destacam entre as principais falhas protéticas: fratura na prótese provisória de acrílico, descolamento de dentes de próteses provisórias e definitivas, afrouxamento e fratura dos parafusos da prótese e pilares, fratura de pontos e solda e da barra metálica; entre as falhas biológicas observa-se: falta de espaço para higienização, má adaptação entre o pilar protético e plataforma do implante, fístulas, parestesia transitória, inflamação periimplatar, hiperplasias, reabsorções ósseas e mucosites. As principais queixas citadas pelos pacientes foram: dificuldades de higienização, mau cheiro, afrouxamento de parafusos, dentes que descolam da prótese e dificuldades na fonação. Também foi possível observar que ouve melhora nas taxas de sucesso nas últimas décadas. Conclui-se que, mesmo com as altas taxas de sucesso do tratamento com as próteses totais fixas sobre implantes, as falhas quando existentes trazem transtornos para o paciente e cirurgião-dentista, o que faz com que se torne importante o estudo das mesmas para aperfeiçoar cada vez mais os protocolos clínicos e prever quando há maiores chances das falhas acontecerem, para que paciente e profissional estejam cientes dos riscos que assumem ao escolherem o tratamento.

One of the challenges of dentistry is the rehabilitation of esthetics, function and health to the edentulous patient. Among the treatment options for total fixed prosthesis on implants emerges as a highly predictable alternative, nonetheless, continues to show small percentages of failures, that even minority, bring to the patients suffering with them great inconvenience and dissatisfaction. The aim of this study was to analyze the literature what are the most common biological and mechanical failures encountered and what are the most frequent complaints in patients with fixed prostheses on implants. The research was done in databases such as SciELO, PubMed, besides the regular Capes portal. Articles published in English and Portuguese between 2006 and 2013 to the writing of this review were selected. After reviewing the literature was possible observe that stand out among the leading prosthetic failure: fracture prosthesis acrylic teeth detachment of provisional and definitive prostheses, prosthesis and screws and pillars loosening and fracture, fracture points and solder the metal bar; among biological failures include: lack of space for hygiene, poor fit between the abutment and the implant platform, fistulas, transient paresthesia, periimplatar inflammation, hyperplasia, bone resorption and mucositis; the main complaints cited by patients were: difficulties hygiene, smelly, loosening of screws, take off the prosthetic teeth, difficulty in speaking. Was also observed that there was a improvement in success rates in recent decades. It was concluded that even with high rates of successful treatment with total fixed prosthesis on implant, failures when existing bring great inconvenience to the patient and the dentist, which means it becomes important to study the same that can increasingly improve clinical protocols and predict when there are greater chances of failures happen, so patient and professional are aware of the risks they take when choosing treatment.
